Why this applies to Erythromycin Stearate
Erythromycin Stearate falls under CYTOCHROME P450 1A2 (CYP1A2) INHIBITORS — the group Guarana interacts with.
Guarana contains caffeine. Caffeine is metabolized by the CYP1A2 enzyme. Theoretically, drugs that inhibit CYP1A2 may decrease the rate of caffeine clearance and increase caffeine levels.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing to know if you take guarana with erythromycin. Guarana is basically a concentrated source of caffeine. Your body clears caffeine using an enzyme called CYP1A2, and erythromycin can mildly slow that enzyme down. So in theory, the caffeine from guarana may stick around a bit longer and build up more than usual.
What that could feel like is too much caffeine: jitteriness, a racing or pounding heart, trouble sleeping, or feeling anxious. I want to be honest though, this is rated minor and is based mostly on lab-type evidence, not strong human studies. A normal cup of coffee likely won't be a problem, but loading up on guarana energy supplements is where you'd notice it more. If you're sensitive to caffeine, just chat with your pharmacist.

Guarana is an Amazonian seed that is naturally high in caffeine, which explains most of its stimulant and energy effects. While it may give a short-term boost in alertness and reduce fatigue, strong evidence for weight loss or major performance gains is limited, and it carries the same risks as too much caffeine. What Is Erythromycin Used For?
Brand names: Erythro-S, Erythrocin, Ethril
Erythromycin treats infections caused by susceptible bacteria. The oral products (delayed-release tablets, tablets and suspensions) are labeled for mild to moderate upper respiratory tract infections caused by Streptococcus pyogenes, Streptococcus pneumoniae and Haemophilus influenzae. For H. influenzae, it is used together with adequate doses of sulfonamides.
Erythromycin lactobionate injection (Erythrocin Lactobionate and other lactobionate products) is used for similar infections, including lower respiratory tract infections, when oral treatment is not possible or when the infection needs high blood levels right away. Topical erythromycin 2% solution, Ery 2% pads and Spotex gel are used on the skin to treat acne vulgaris.
