Exenatide falls under ANTIDIABETES DRUGS — the group Guarana interacts with.
Some conflicting reports claim that caffeine, a constituent of guarana, might increase or decrease blood sugar.

Here's the simple version for guarana and exenatide (Byetta, Bydureon). The caffeine in guarana doesn't fight with exenatide directly. The concern is more roundabout: caffeine can nudge your blood sugar a little, and the reports go both ways, with some saying it raises sugar and others saying it lowers it. So in theory it could make your numbers a touch harder to predict while you're on exenatide.
Honestly, this is a minor, mostly theoretical worry based on weak evidence, so a regular cup of coffee isn't likely to cause trouble. The bigger thing to watch is that loading up on concentrated guarana or energy products can mask early low-blood-sugar warning signs (shakiness, racing heart) since caffeine causes similar feelings. If you use it, keep checking your sugar and chat with your pharmacist or doctor first.
