Ephedrine Sulfate falls under STIMULANT DRUGS — the group Guarana interacts with.
Guarana contains caffeine. Due to the central nervous system (CNS) stimulant effects of caffeine, concomitant use with stimulant drugs can increase the risk of adverse effects.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version: guarana is basically a natural caffeine source, and ephedrine is itself a stimulant. So when you stack the two, you're piling one stimulant on top of another. Both of them rev up your nervous system, so the worry is that their effects add together. This isn't a case of one changing how the other is broken down. It's just two energizers working at the same time.
What that can feel like is a faster or pounding heartbeat, higher blood pressure, jitteriness, trouble sleeping, headache, or feeling anxious and shaky. This concern is mostly theoretical, based on how the two work rather than hard studies, so don't panic. Still, since ephedrine already affects your heart, please check with your pharmacist or doctor before pairing them.

Guarana contains caffeine. Theoretically, concomitant use of large amounts of caffeine might increase cardiac inotropic effects of beta-agonists.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the simple version: guarana is loaded with caffeine, and ephedrine is itself a stimulant that revs up your heart and blood pressure. Stack the two together and you're basically piling one stimulant on top of another. In theory, the caffeine can amplify ephedrine's punch on the heart, meaning a stronger or faster heartbeat, a bigger bump in blood pressure, plus jitteriness, anxiety, trouble sleeping, or a feeling like your heart is pounding.
I want to be honest about the evidence though. This is a theoretical concern based on how these stimulants work, not strong proof from studies, and a normal sip of guarana soda is far gentler than concentrated supplement doses. Still, since both push the heart, it's worth running past your doctor or pharmacist before combining them, especially if you have any heart or blood pressure issues.
