Clarithromycin falls under CYTOCHROME P450 1A2 (CYP1A2) INHIBITORS — the group Guarana interacts with.

Theoretically, concomitant use might increase the levels and adverse effects of caffeine.
Guarana contains caffeine. Caffeine is metabolized by the CYP1A2 enzyme. Theoretically, drugs that inhibit CYP1A2 may decrease the rate of caffeine clearance and increase caffeine levels.

Here's the thing to keep in mind with guarana and clarithromycin: guarana is basically concentrated caffeine, and clarithromycin can mildly slow down the liver enzyme (CYP1A2) that clears caffeine from your body. When that happens, caffeine can hang around longer and build up a bit, so you may feel the usual caffeine overload symptoms, things like jitteriness, a racing or pounding heartbeat, trouble sleeping, or feeling on edge.

I want to be honest about how solid this is. The interaction is rated minor and is mostly theoretical, based on lab data rather than real patient reports, so it's not something to panic over. Still, if you notice you feel extra wired while on this antibiotic, ease up on guarana and other caffeine sources, and check in with your pharmacist or doctor.

About Clarithromycin

What Is Clarithromycin Used For?

Brand names: Biaxin, Biaxin XL

Clarithromycin tablets and oral suspension treat a range of mild to moderate bacterial infections in adults and children, including community-acquired pneumonia, acute sinus infections, acute bacterial flare-ups of chronic bronchitis, strep throat and tonsillitis, uncomplicated skin infections, and middle ear infections in children. Tablets are also used in adults to eliminate H. pylori bacteria (in combination with other medicines) to reduce the risk of duodenal ulcer recurrence.

Clarithromycin tablets — but not the extended-release tablet — are also used to treat and prevent disseminated Mycobacterium avium complex (MAC) infections in people with advanced HIV. Clarithromycin extended-release tablets are approved only for acute bacterial exacerbation of chronic bronchitis, acute maxillary sinusitis, and community-acquired pneumonia in adults — not for the other conditions listed above.
