Iloprost falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Guarana interacts with.

Theoretically, guarana may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
In vitro and animal research suggests that guarana extract can inhibit platelet aggregation. This effect may be due to the caffeine in guarana, which is also reported to have antiplatelet activity. This interaction has not been reported in humans.

Here's the thing to know about pairing guarana with iloprost (Ventavis). Iloprost not only relaxes blood vessels in your lungs, it also makes platelets (the cells that help your blood clot) a little less sticky. Guarana, mostly because of its caffeine, has shown a similar mild anti-clotting effect in lab and animal studies. Stacked together, the theory is they could add up and slightly raise your bleeding risk.

That said, this is theoretical and hasn't actually been reported in people, so I wouldn't panic. Just keep an eye out for easy bruising, nosebleeds, or bleeding gums, and mention the guarana (or any energy drinks and caffeine pills) to your pharmacist or doctor before adding it.

Aurlumyn (iloprost injection, given intravenously) is approved to treat severe frostbite in adults, specifically to reduce the risk of losing fingers or toes (digit amputations). Its effectiveness was established in young, healthy adults who experienced frostbite at high altitudes.
