Zileuton falls under CYTOCHROME P450 1A2 (CYP1A2) INHIBITORS — the group Guarana interacts with.

Theoretically, concomitant use might increase the levels and adverse effects of caffeine.
Guarana contains caffeine. Caffeine is metabolized by the CYP1A2 enzyme. Theoretically, drugs that inhibit CYP1A2 may decrease the rate of caffeine clearance and increase caffeine levels.

Here's the thing to know about pairing guarana with Zileuton: it's really the caffeine in guarana that's the concern, not your asthma medicine itself. Zileuton can slow down the CYP1A2 enzyme, which happens to be the same enzyme your body uses to clear caffeine. So when you take them together, caffeine may hang around longer and build up a bit more than usual.

In plain terms, your usual cup of energy could hit harder. You might feel jittery, anxious, have a racing heart, or struggle to sleep. The good news is this is rated minor and is only based on lab-type reasoning, not strong human studies. Casual amounts are unlikely to be a big deal, but if you take concentrated guarana and notice these effects, check with your pharmacist or doctor.

About Zileuton

What Is Zileuton Used For?

Brand names: Zyflo, Zyflo CR

Zileuton is used for the ongoing prevention and treatment of asthma in adults and adolescents aged 12 and older. It is a maintenance medicine — meaning it is taken regularly to keep asthma under control, not to stop an asthma attack once it has started.

Zyflo (the tablet form) and zileuton extended-release tablets are both approved for this same purpose. Neither form should be used to treat a sudden asthma attack. However, both can be continued during a flare-up of asthma while you also use your rescue inhaler.
