Here's the gist for your asthma med: St. John's wort speeds up the liver enzyme (CYP3A4) that helps clear zileuton. When that enzyme works faster, more of your zileuton gets broken down, so the levels in your blood can drop. Zileuton isn't a prodrug, so lower levels just mean a weaker effect.

In plain terms, your zileuton may not control your asthma as well. You could notice more wheezing, coughing, chest tightness, or shortness of breath creeping back. Since zileuton is a daily preventer, not a rescue inhaler, losing that protection matters. The evidence here is real but moderate, so don't panic. Just talk with your pharmacist or doctor before combining these, and let them know if your breathing changes.

Here's the gist for Zileuton (Zyflo): your body relies on a liver enzyme called CYP1A2 to clear this medicine. St. John's wort can rev up that enzyme, so it may push Zileuton out of your system faster than usual. Zileuton is an active drug, not a prodrug, so faster clearance means less of it sticking around to do its job.

In practice, that could mean your asthma is less well controlled, with more wheezing, coughing, chest tightness, or shortness of breath creeping back. St. John's wort may increase the metabolism and reduce the levels of CYP2C9 substrates.
There is contradictory research about the effect of St. John's wort on CYP2C9. Some in vitro research shows that St. John's wort induces CYP2C9, but to a lesser extent than CYP3A4. St. John's wort also induces metabolism of the S-warfarin isomer, which is a CYP2C9 substrate. Other research shows that St. John's wort 300 mg three times daily for 21 days does not significantly affect the pharmacokinetics of a single 400 mg dose of ibuprofen, which is also a CYP2C9 substrate. Until more is known, use St. John's wort cautiously in patients who are taking CYP2C9 substrates.

Here's the gist for your asthma medicine. Zileuton is partly cleared by a liver enzyme called CYP2C9, and St. John's wort can nudge that enzyme to work faster. If your body breaks down zileuton more quickly, blood levels can drop, which may mean it does a weaker job preventing your asthma symptoms. In real life that could show up as more wheezing, coughing, chest tightness, or shortness of breath sneaking back over time.

That said, the evidence here is mixed. Some studies show St. John's wort speeds up CYP2C9, while others show little effect, so this is a possible, not a guaranteed, problem. Since uncontrolled asthma is nothing to gamble with, please check with your pharmacist or doctor before combining these rather than changing anything on your own.

About Zileuton

What Is Zileuton Used For?

Brand names: Zyflo, Zyflo CR

Zileuton is used for the ongoing prevention and treatment of asthma in adults and adolescents aged 12 and older. It is a maintenance medicine — meaning it is taken regularly to keep asthma under control, not to stop an asthma attack once it has started.

Zyflo (the tablet form) and zileuton extended-release tablets are both approved for this same purpose. Neither form should be used to treat a sudden asthma attack. However, both can be continued during a flare-up of asthma while you also use your rescue inhaler.
