Riluzole falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group St. John's Wort interacts with.

St. John's wort may increase the metabolism and reduce the levels of CYP1A2 substrates.
Clinical and in vitro research shows that St. John's wort induces CYP1A2, but to a lesser extent than CYP3A4.

Here's the gist for ALS treatment. Your body clears riluzole mainly through a liver enzyme called CYP1A2. St. John's wort can speed that enzyme up, which means riluzole could get broken down faster and leave your bloodstream sooner. Lower levels can mean you're not getting the full benefit of the medication, and for a drug used to slow ALS, that's something you really don't want to lose.

Riluzole works as-is (it isn't a prodrug), so faster clearance simply means a weaker effect, not stronger side effects. This effect is considered possible rather than certain, but given how important this medicine is, please don't add St. John's wort without first checking with your pharmacist or neurologist.

What Is Riluzole Used For?

Brand names: Rilutek, Tiglutik, Exservan

Riluzole is approved to treat amyotrophic lateral sclerosis (ALS) — the progressive neurological disease also known as Lou Gehrig's disease. This indication is shared across all available riluzole products, including tablets (Rilutek, riluzole), the dissolving oral film (Exservan), and the oral suspension (Tiglutik).

Tiglutik oral suspension is notable in that it can also be given through a PEG-tube (percutaneous endoscopic gastrostomy tube) for patients who have difficulty swallowing.
