St. John's wort can increase oxycodone metabolism by inducing cytochrome P450 3A4 (CYP3A4), reducing plasma levels and analgesic activity.

Here's the short version: St. John's wort can make your oxycodone work less well. The herb speeds up a liver enzyme called CYP3A4, which is one of the main pathways your body uses to clear oxycodone. When that pathway runs faster, more of your dose gets broken down before it can ease your pain.

In real life, that can mean your usual dose just doesn't take the edge off like it used to. The worry isn't more side effects here, it's the opposite: pain that creeps back, and the temptation to take extra to catch up. A lower quality study backs this up, so it's a real concern, not just theory. Please don't change anything on your own. Loop in your pharmacist or doctor first so they can keep your pain controlled safely.

Oxycodone is prescribed for pain severe enough to need daily, long-term opioid treatment — pain that hasn't responded well to non-opioid medicines or shorter-acting options. The extended-release form is designed for continuous pain control, not for occasional or as-needed use.

It is approved for adults and, in certain cases, for opioid-tolerant children 11 years and older who are already on opioid therapy. Because of the serious risks involved, it is reserved for situations where other approaches have genuinely fallen short.
