Here's the concern with combining these two. Your body clears levonorgestrel using a liver enzyme called CYP3A4. St. John's wort speeds that enzyme up, so it can flush the hormone out of your system faster than normal. The end result is lower levonorgestrel levels in your blood.

For an emergency contraceptive like Plan B, that's a real problem, because weaker levels may mean it doesn't work well enough to prevent pregnancy. The same worry applies to ongoing forms like Mirena or Skyla. This isn't theoretical; it's been seen in studies, and St. John's wort products with meaningful hyperforin are the ones that matter. Please talk with your pharmacist or doctor about a backup or alternative before relying on this combination.

Levonorgestrel also falls under CONTRACEPTIVE DRUGS.

St. John's wort increases the clearance of contraceptive drugs and reduces their clinical effects.
Females taking St. John's wort and oral contraceptives concurrently should use an additional or alternative form of birth control. St. John's wort can decrease norethindrone and ethinyl estradiol levels by 13% to 15%, resulting in breakthrough bleeding, irregular menstrual bleeding, or unplanned pregnancy. Bleeding irregularities usually occur within a week of starting St. John's wort and regular cycles usually return when St. John's wort is discontinued. Unplanned pregnancy has occurred with concurrent use of oral contraceptives and St. John's wort extract. St. John's wort is thought to induce the cytochrome P450 1A2 (CYP1A2), 2C9 (CYP2C9), and 3A4 (CYP3A4) enzymes, which are responsible for metabolism of progestins and estrogens in contraceptives.

Here's the key thing to know: St. John's wort revs up the liver enzymes (mainly CYP3A4) that break down progestins like levonorgestrel. When those enzymes work faster, your body clears the hormone more quickly, so less of it stays active. For an emergency contraceptive pill (like Plan B), that's a real concern, because you only get one shot at preventing pregnancy and you need full strength to do it.

With Plan B specifically, taking St. John's wort could make it less reliable, raising the chance it fails. For levonorgestrel IUDs (Mirena, Skyla) the local effect is less likely to be undone, but it's worth confirming. Please don't start or stop anything on your own. Talk with your pharmacist or doctor so they can recommend a backup or alternative method.

About Levonorgestrel

What Is Levonorgestrel Used For?

Brand names: Plan B, Mirena, Levonelle One Step, Levonelle 1500, Norgeston, Skyla, Liletta, Kyleena

Levonorgestrel is used in two very different ways depending on the product. As an intrauterine system (IUS), Mirena and Liletta prevent pregnancy for up to 8 years and also treat heavy menstrual bleeding for up to 5 years in people who choose intrauterine contraception. Kyleena prevents pregnancy for up to 5 years, and Skyla prevents pregnancy for up to 3 years — neither Kyleena nor Skyla is indicated for heavy menstrual bleeding.

As an oral tablet, products such as AfterPill, AfterPlan, Amazon Basic Care Levonorgestrel, Econ Morning After, Emergency Contraceptive, and Empowered Choice are used as emergency (backup) contraception to reduce the chance of pregnancy after unprotected sex or contraceptive failure. The oral tablet is not intended for routine birth control.
