Here's the gist for hydrocortisone (oral or injected forms like Cortef): your liver clears this steroid using an enzyme called CYP3A4. St. John's wort speeds that enzyme up, so it can flush hydrocortisone out of your body faster. That means you may end up with less steroid working than your doctor intended.

If you take hydrocortisone to replace a hormone your body lacks (such as adrenal insufficiency), this matters most, because weaker levels can leave you tired, weak, lightheaded, or even very sick during stress or illness. For arthritis or allergy use, you might just notice your symptoms aren't as well controlled. Please don't change anything on your own. Talk with your pharmacist or doctor first, especially before starting or stopping the herb. Skin creams are far less likely to be affected.

St. John's wort decreases the levels and clinical effects of P-glycoprotein substrates.
St. John's wort induces P-glycoprotein. P-glycoprotein is a carrier mechanism responsible for transporting drugs and other substances across cell membranes. When P-glycoprotein is induced in the gastrointestinal (GI) tract, it can prevent the absorption of some medications. In addition, induction of p-glycoprotein can decrease entry of drugs into the central nervous system (CNS) and decrease access to other sites of action.

Here's the gist of this combo: St. John's wort revs up a transporter called P-glycoprotein, which acts like a little pump that moves drugs in and out of cells. When that pump speeds up, less of your hydrocortisone may get absorbed and stay around, so the steroid could end up working less than expected.

Why that matters depends on why you take it. If you rely on hydrocortisone to replace hormones your body doesn't make (like for adrenal insufficiency), a weaker effect could leave you feeling tired, dizzy, weak, or run-down. If you take it for inflammation, arthritis, or an immune problem, your symptoms might flare back up. The evidence here isn't airtight, but the interaction is considered likely, so please don't start St. John's wort without checking with your pharmacist or doctor first.

What Is Hydrocortisone Used For?

Brand names: Cortef, Hydrocortone, Carmol HC cream, Hydrocortisone Iodoquinol Cream, Vytone Cream, Massengil Medicated Towelettes, Dioderm, Efcortelan Cream, Mildison, Alphaderm, Calmurid HC, Efcortelan Ointment, Texacort, Alkindi Sprinkle

Hydrocortisone is used to calm skin inflammation and relieve itching caused by conditions that respond to corticosteroid treatment. In adults, it treats a broad range of inflammatory skin conditions. In children as young as 3 months old, it is specifically approved for mild to moderate atopic dermatitis (eczema).

It is also used in rectal foam form to treat inflammation of the lower bowel, such as ulcerative proctitis.
