Theoretically, St. John's wort might inhibit reuptake and increase levels of serotonin, resulting in additive effects with serotonergic drugs.
Concomitant use of serotonergic drugs with St. John's wort can lead to increased adverse effects and increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstriction disorders.

Here's the concern when you pair St. John's wort with sumatriptan (Imitrex). Both can raise serotonin activity in the body. Sumatriptan works on serotonin receptors to ease migraines, and St. John's wort is thought to boost serotonin levels too. Stacking them could add up and, in rare cases, push serotonin too high.

That's what's called serotonin syndrome, with signs like agitation, a racing heart, sweating, shivering, muscle twitching, or confusion. I'll be honest though: this warning is mostly theoretical, based on how the two work rather than lots of real-world reports. Still, it's worth a quick conversation with your pharmacist or doctor before combining them, especially if you take sumatriptan often.
