Dexamethasone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group St. John's Wort interacts with.
St. John's wort induces CYP3A4 enzymes and increases metabolism of CYP3A4 substrates. Clinically significant interactions have been reported with St. John's wort products containing hyperforin 1 mg or more.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the short version for this pair: your body uses an enzyme called CYP3A4 to break down dexamethasone. St. John's wort revs that enzyme up, so it clears the steroid out faster. The result is less dexamethasone working in your system, not more.
Why that matters depends on why you're taking it. If you rely on dexamethasone for something serious (controlling inflammation, an autoimmune flare, hormone replacement, nausea with chemo, or as part of a cancer plan), weaker steroid levels could mean your symptoms come back or the medicine doesn't do its job. This has been seen in real studies, so it's worth taking seriously. Please don't change anything on your own. Talk with your pharmacist or doctor before combining these two.

P-GLYCOPROTEIN SUBSTRATES

St. John's wort induces P-glycoprotein. P-glycoprotein is a carrier mechanism responsible for transporting drugs and other substances across cell membranes. When P-glycoprotein is induced in the gastrointestinal (GI) tract, it can prevent the absorption of some medications. In addition, induction of p-glycoprotein can decrease entry of drugs into the central nervous system (CNS) and decrease access to other sites of action.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the gist when you pair St. John's wort with dexamethasone. This herb revs up a transporter called P-glycoprotein, and it also speeds up liver enzymes that help clear steroids. The practical worry is that St. John's wort can make your body push out and break down dexamethasone faster, so less of the drug stays active in your system.
For you, that can mean the dexamethasone simply doesn't work as well. If you're relying on it to control inflammation, an allergic flare, asthma, or as part of cancer treatment, weaker steroid coverage is a real concern. Clinical research shows that taking St. John's wort 325 mg three times daily for 14 days along with bupropion, a CYP2B6 substrate, reduces the area under the concentration-time curve by approximately 14% and increases the clearance of bupropion by approximately 20%.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: St. John's wort acts like a speed-up button for certain liver enzymes, including one called CYP2B6 that helps process dexamethasone. When that enzyme works faster, your body can clear this steroid more quickly, which may leave less of it in your system to do its job.
In practical terms, that could mean dexamethasone doesn't control your inflammation, allergy, or other condition as well as expected. If you're taking it for something serious like cancer treatment or adrenal replacement, even a partial drop in effect matters. The evidence here comes from a related drug (bupropion) and the change was modest, so this is a possible concern rather than a sure thing. Please don't start or stop anything on your own. What Is Dexamethasone Used For?

Dexamethasone is mainly used to calm inflammation and overactive immune responses. Depending on the product, it treats allergic conditions, skin diseases, adrenal and other hormone disorders, and certain bowel diseases.
Some uses belong only to specific products. Hemady tablets are used with other anti-myeloma medicines for adults with multiple myeloma. Dextenza (an eye insert) treats eye inflammation and pain after eye surgery and eye itching from allergic conjunctivitis. Ozurdex (an eye implant) treats macular edema after retinal vein occlusion, non-infectious uveitis of the back of the eye, and diabetic macular edema. Maxidex and dexamethasone sodium phosphate eye drops treat steroid-responsive inflammation of the front of the eye.
