Here's the thing to keep in mind with this pair. St. John's wort revs up a liver enzyme called CYP3A4, which speeds up how fast your body clears certain drugs. On paper, miconazole is listed as a CYP3A4 substrate, so the worry is that St. John's wort could lower its levels and make it work less well.

In real life, though, this matters far less for the way most people use miconazole. Products like Monistat creams, vaginal suppositories, and antifungal powders work right where you apply them and barely get into your bloodstream, so St. John's wort isn't likely to affect them. The bigger picture is that miconazole itself is a strong CYP3A4 blocker. If you ever take an oral form, please check with your pharmacist or doctor first.
