Ciprofloxacin falls under CYTOCHROME P450 1A2 (CYP1A2) INHIBITORS — the group Guarana interacts with.

Theoretically, concomitant use might increase the levels and adverse effects of caffeine.
Guarana contains caffeine. Caffeine is metabolized by the CYP1A2 enzyme. Theoretically, drugs that inhibit CYP1A2 may decrease the rate of caffeine clearance and increase caffeine levels.

Here's the thing to know if you're taking Cipro and reaching for a guarana energy product: guarana is basically concentrated caffeine, and ciprofloxacin slows down the liver enzyme (CYP1A2) that normally clears caffeine from your body. With that cleanup process running slower, caffeine can hang around longer and build up. So that one guarana drink might hit you harder than usual, with more jitteriness, a racing or pounding heart, trouble sleeping, or feeling anxious and shaky.

Honestly, this is mostly a comfort issue, and the evidence here is theoretical (based on lab work, not strong human studies). Still, while you're on Cipro it's smart to go easy on guarana and other caffeine. If you feel wired or your heart is pounding, check in with your pharmacist or doctor.

Guarana contains caffeine. Theoretically, concomitant use might increase serum caffeine concentrations and the risk of adverse effects. Quinolones (also called fluoroquinolones) can decrease caffeine clearance by inhibiting the cytochrome P450 1A2 enzyme.

Here's the thing to know if you're on Cipro and reaching for guarana: this one is really about the caffeine. Cipro slows down a liver enzyme called CYP1A2, and that same enzyme is what your body uses to clear caffeine. Since guarana is loaded with natural caffeine, taking the two together can let caffeine hang around longer and build up more than usual.

In plain terms, the guarana may hit harder and last longer. You might notice a jittery feeling, a racing or pounding heart, trouble sleeping, or more anxiety. This is rated a minor and mostly theoretical interaction, so it's not dangerous for most people, but you may simply feel like you've had way too much coffee. If you're sensitive to caffeine or have heart or anxiety issues, check with your pharmacist or doctor first.

Ciprofloxacin belongs to a class of antibiotics called fluoroquinolones. It's prescribed to treat bacterial infections in many parts of the body — including the urinary tract, skin, lungs, bones, joints, and digestive system. It's also used to protect against or treat anthrax and plague after exposure.

Because ciprofloxacin carries real risks, it's reserved for milder conditions like sinus infections, simple bladder infections, and flare-ups of chronic bronchitis only when there are no safer alternatives available.
