Hydrochlorothiazide, Methyldopa falls under DIURETIC DRUGS — the group Guarana interacts with.

Theoretically, using guarana with diuretic drugs might increase the risk of hypokalemia.
Guarana contains caffeine. Caffeine, especially in excessive amounts, can reduce potassium levels due to stimulation of the sodium-potassium pump. Diuretics can also lower potassium levels.

Here's the deal with this combo. Your medication contains hydrochlorothiazide, a water pill that already nudges potassium out of your body through your urine. Guarana is basically concentrated caffeine, and in larger amounts caffeine can also push potassium down by revving up the pump that moves it into your cells. Stack the two together and, in theory, your potassium could dip a little lower than it would otherwise.

Honestly, this is more of a theoretical concern than something proven, and an occasional energy drink probably won't tip you over. But if you're loading up on guarana or high-caffeine supplements, watch for muscle cramps, weakness, or an odd heartbeat. The methyldopa part of your pill isn't the issue here. Check with your pharmacist or doctor before adding guarana regularly.
