Amiloride, Hydrochlorothiazide falls under DIURETIC DRUGS — the group Guarana interacts with.

Theoretically, using guarana with diuretic drugs might increase the risk of hypokalemia.
Guarana contains caffeine. Caffeine, especially in excessive amounts, can reduce potassium levels due to stimulation of the sodium-potassium pump. Diuretics can also lower potassium levels.

Here's something worth knowing about your water pill. The hydrochlorothiazide half of this combo can lower your potassium, while the amiloride half is actually a 'potassium-sparing' diuretic added to help hold onto it. That balance is the whole point of the combo. Now, guarana is basically concentrated caffeine, and in large amounts caffeine can nudge potassium down a little by pushing it into your cells.

To be honest, this concern is theoretical and based on how caffeine behaves, not on strong studies with this exact medicine. The amiloride piece works in your favor here. Still, if you notice muscle cramps, weakness, or an irregular heartbeat, or you're using high-dose guarana regularly, check in with your pharmacist or doctor.

What Is Amiloride and Hydrochlorothiazide Used For?

Brand names: Moduretic, Amilzide, Moduret 25, Amil-Co

Amiloride hydrochloride and hydrochlorothiazide tablets are used for patients with high blood pressure (hypertension) or congestive heart failure who tend to develop low potassium (hypokalemia) when taking a thiazide or other diuretic alone — or for whom keeping potassium levels in a normal range is especially important, such as people taking digoxin or those with significant heart rhythm problems.

The tablets may be used on their own or alongside other blood pressure medicines, such as methyldopa or beta blockers. This combination product is not intended as the first-choice treatment for swelling (edema) or high blood pressure unless low potassium is a specific concern.
