Guarana and Alogliptin, Metformin

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Guarana interacts with whole categories of medication. Alogliptin, Metformin falls into 2 of those categories, so it can interact with Guarana in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Minor).

Minor

ANTIDIABETES DRUGS

1 of 2

Why this applies to Alogliptin, Metformin

Alogliptin, Metformin falls under ANTIDIABETES DRUGS — the group Guarana interacts with.

Theoretically, taking guarana with antidiabetes drugs might interfere with blood glucose control.
Some conflicting reports claim that caffeine, a constituent of guarana, might increase or decrease blood sugar.
Severity Minor Likelihood Unlikely Evidence B

Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the short version: guarana is basically a concentrated source of caffeine, and your Kazano (alogliptin plus metformin) is what keeps your blood sugar in a steady range. The concern on file is that caffeine might nudge your blood sugar a little, but honestly the reports go both ways. Some suggest it could raise it slightly, others suggest it could lower it, so the direction isn't even consistent.

This is rated minor and unlikely, and the evidence behind it is weak, so I wouldn't lose sleep over a normal cup of coffee or tea. What I'd watch for is the bigger picture: a lot of caffeine can mask early low-sugar warning signs like shakiness or a racing heart. If you take an energy supplement regularly, just check your glucose a bit more closely and run it by your pharmacist or doctor first.

METFORMIN (Glucophage)

2 of 2
Theoretically, metformin might increase the levels and adverse effects of caffeine.
Guarana contains caffeine. Animal research shows that metformin can reduce caffeine metabolism. However, this effect has not been reported in humans.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to keep in mind with Kazano (which combines alogliptin and metformin) and guarana: this isn't about your diabetes medicine becoming weaker or stronger. It's the other way around. The metformin piece may slow how your body clears caffeine, and guarana is basically a concentrated source of caffeine. So the caffeine could hang around a bit longer than usual.

In real life that might mean more of the usual caffeine stuff, like jitteriness, a faster or pounding heartbeat, trouble sleeping, or feeling on edge. I'll be honest with you though, this is only seen in animal studies and hasn't actually shown up in people, so it's pretty theoretical. A cup of coffee likely won't matter much; concentrated guarana pills are where I'd pay attention. If you're sensitive to caffeine, check with your pharmacist or doctor.

From our pharmacist’s overview on the Alogliptin, Metformin page

“Kazano combines alogliptin and metformin to lower blood sugar, and nearly everything on this list pushes on that same lever, so I treat these pairings with real caution. Most entries are herbs and supplements that lower glucose on their own, and stacked on the drug they raise the risk of hypoglycemia, a sugar drop steep enough to cause…”

About Guarana

What is Guarana?

Guarana
  • Energy and alertness
  • Reducing fatigue
  • Weight loss support
  • Athletic performance
  • Mental focus

Guarana is an Amazonian seed that is naturally high in caffeine, which explains most of its stimulant and energy effects. While it may give a short-term boost in alertness and reduce fatigue, strong evidence for weight loss or major performance gains is limited, and it carries the same risks as too much caffeine. Talk to your pharmacist or doctor before using it, especially if you have heart problems, anxiety, or are pregnant.
