The piece of this combo product that matters here is the phenylephrine, which is a decongestant that works as a mild stimulant. It can already nudge up your heart rate and blood pressure and leave some people feeling jittery or wired. Guarana is basically a concentrated source of caffeine, which is also a stimulant. Stack the two together and those effects can add up.

So in plain terms, pairing them may increase stimulant-type side effects like a racing or pounding heart, restlessness, trouble sleeping, headache, or feeling anxious. This is a theoretical concern based on how both work, not a hard-proven danger, so it's not a reason to panic. Still, if you have high blood pressure, heart issues, or anxiety, check with your pharmacist or doctor first.
