Sacubitril, Valsartan falls under DIURETIC DRUGS — the group Guarana interacts with.

Theoretically, using guarana with diuretic drugs might increase the risk of hypokalemia.
Guarana contains caffeine. Caffeine, especially in excessive amounts, can reduce potassium levels due to stimulation of the sodium-potassium pump. Diuretics can also lower potassium levels.

Here's the thing to know about pairing guarana with Entresto (sacubitril/valsartan). Guarana's main ingredient is caffeine, and in large amounts caffeine can nudge potassium out of your blood. On the surface that might raise a flag, since some heart-failure regimens involve water pills that also lower potassium.

But Entresto itself works a bit differently. The valsartan part actually tends to hold onto potassium, so this combo more often leans toward potassium being too high than too low. This guarana concern is theoretical, based on how caffeine behaves rather than real-world reports. A regular cup of coffee or tea is unlikely to matter, but heavy guarana energy products could. Since you're managing heart failure, please check with your pharmacist or doctor before adding guarana.

What Is Valsartan and Sacubitril Used For?

Brand name: Entresto

Sacubitril and valsartan is used to reduce the risk of cardiovascular death and hospitalization for heart failure in adult patients with chronic heart failure. The benefit is most clearly seen in patients whose heart pumps with a lower-than-normal ejection fraction (the percentage of blood the heart pushes out with each beat).

It is also approved to treat symptomatic heart failure with reduced left ventricular function in children aged 1 year and older. In pediatric patients, it lowers a blood marker of heart stress (NT-proBNP) and is expected to improve heart outcomes.
