Aspirin, Caffeine, Orphenadrine falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Guarana interacts with.
In vitro and animal research suggests that guarana extract can inhibit platelet aggregation. This effect may be due to the caffeine in guarana, which is also reported to have antiplatelet activity. This interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's something worth knowing if you take this combination product. The big concern is the aspirin part, which already thins your blood by blocking platelets from clumping together. Lab and animal studies hint that guarana may do something similar, likely because of its caffeine. Stacked on top of aspirin, the theory is you could nudge your bleeding risk a little higher, things like easier bruising, nosebleeds, or a cut that's slow to stop.
I want to be honest though: this has not actually been seen in people, so it stays theoretical for now. There's also a second wrinkle, since this product already contains caffeine, adding caffeine-heavy guarana can pile on jitteriness, a racing heart, or trouble sleeping. Check with your pharmacist before combining them.

Guarana contains caffeine. Due to the central nervous system (CNS) stimulant effects of caffeine, concomitant use with stimulant drugs can increase the risk of adverse effects.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version: this combo product (think Norgesic or Orphengesic) already contains caffeine as one of its three ingredients. Guarana is basically a natural caffeine source, so taking the two together is like stacking caffeine on top of caffeine. You're not changing how the medicine is broken down, you're just adding more of the same stimulant.
Realistically, that can mean a jittery, wired feeling, a faster or pounding heartbeat, trouble sleeping, anxiety, or an upset stomach. This is a theoretical concern based on how caffeine works, and a little guarana in a drink probably won't cause much. But concentrated guarana supplements are a different story. Before combining them, check with your pharmacist or doctor.
