Aspirin, Caffeine, Orphenadrine falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Danshen interacts with.

Theoretically, danshen may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
Danshen has been reported to have antithrombotic effects. Animal research also suggests that taking a danshen combination formula with clopidogrel exhibits a synergistic increase in antiplatelet aggregation and prolongation of coagulation time when compared with either taken alone.

The piece of this combo pill that matters most here is the aspirin. Aspirin already thins your blood a bit by blocking platelets from clumping together. Danshen seems to do something similar, so taking the two together may stack their effects and raise your chance of bleeding or bruising more easily. Watch for things like nosebleeds, bleeding gums, dark or bloody stools, or bruises that show up without a good reason.

To be fair, this concern is based mostly on animal studies and scattered case reports, not strong human trials, so it isn't certain. Still, since bleeding can be serious, it's worth a quick chat with your pharmacist or doctor before pairing danshen with this medicine. Please don't stop either one on your own.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research shows that danshen tincture and various constituents of danshen inhibit the activity of CYP1A2. However, this activity has not been shown in humans when theophylline, a CYP1A2 substrate, was used as a target drug.

This combination pill carries caffeine, which your liver clears using an enzyme called CYP1A2. In test-tube studies, Danshen seems to slow that enzyme down. If that played out in people, caffeine could build up a bit and leave you feeling jittery, anxious, or restless, with a racing heart or trouble sleeping. The good news: when researchers tested Danshen with a similar CYP1A2 drug in actual humans, they didn't see this effect, so this is largely a theoretical concern based on lab work, not strong real-world evidence.

One thing worth flagging separately: Danshen may thin the blood, and this product already contains aspirin, so the two together could raise bruising or bleeding risk. I'd run both by your pharmacist or doctor before combining them.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

This combination product (aspirin, caffeine, and orphenadrine) has one part worth thinking about with Danshen: the orphenadrine. Orphenadrine is partly broken down by a liver and gut enzyme called CYP3A4. In small studies, taking Danshen for about 10 to 14 days seemed to speed up that enzyme, which could lower orphenadrine levels and make it work less well for muscle aches. Oddly, a single dose of Danshen did the opposite and raised levels temporarily.

There's also a separate concern: Danshen may thin the blood, and aspirin already does too, so together they could raise your bleeding or bruising risk. The evidence here is early and not strong, so don't panic, but please check with your pharmacist or doctor before mixing them.

Theoretically, danshen may increase the levels of aspirin and the risk of bleeding.
Research in healthy adult males shows that taking a combination of danshen and kudzu with aspirin increases plasma aspirin area under the curve by approximately 3.4-fold. Animal research also shows that taking a combination of danshen and kudzu (danshen-gegen formula) with aspirin increases maximal blood levels of aspirin and salicylic acid by approximately 4-fold and 3.7-fold, respectively, without impacting blood loss. Taking danshen increases the antiplatelet activity of aspirin and might increase the side effects of aspirin.

Your combination product (Norgesic and similar) contains aspirin, and that's the piece Danshen can interact with. Studies in healthy men found that pairing Danshen with aspirin raised aspirin blood levels several times over. On top of that, Danshen on its own can make blood a bit slower to clot. Stacking those two effects can push aspirin's blood-thinning action higher than expected.

In real life, that could mean easier bruising, nosebleeds, stomach upset or irritation, or bleeding that's harder to stop. The caffeine and orphenadrine parts aren't the concern here. Don't change anything on your own. Just check with your pharmacist or doctor first, especially if you already have stomach issues or take other blood thinners.
