Budesonide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the thing with budesonide: your body relies on an enzyme called CYP3A4 to break it down. That's actually a big deal for this steroid, because it already passes through the liver heavily before it ever reaches your bloodstream. Danshen can nudge that enzyme in different directions, and that's where it gets interesting.

In studies, taking Danshen for a week or two seemed to speed up CYP3A4, which could lower budesonide levels and make it work less well (think more Crohn's flares or less control of your kidney inflammation). But a single dose did the opposite, raising levels, which could mean more steroid side effects like fluid retention, mood changes, or higher blood sugar. Since the effect swings both ways and the evidence is early, please run this by your pharmacist or doctor before combining them.

What Is Budesonide Used For?

Brand names: Entocort EC, Pulmicort, Uceris, Tarpeyo, Eohilia

Budesonide inhalation suspension is used as a maintenance (long-term control) treatment for asthma in children 12 months to 8 years old. It also works as a preventive (prophylactic) therapy to reduce the frequency of asthma symptoms.

Important: This medication is not a rescue inhaler. It will not stop a sudden asthma attack. A separate fast-acting bronchodilator is needed for sudden symptoms.
