P-GLYCOPROTEIN SUBSTRATES

Hydrocortisone falls under P-GLYCOPROTEIN SUBSTRATES — the group Danshen interacts with.
Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for hydrocortisone: it's partly moved around the body by a transporter called P-glycoprotein, which acts like a pump that helps push the drug out of cells. Danshen seems to slow that pump down a bit. In a small study, when danshen was taken before a similar pump-dependent drug, blood levels of that drug rose by roughly 27 to 37 percent. So in theory, danshen could let a little more hydrocortisone build up than usual.
If that happens, you might notice more of the typical steroid effects: fluid retention, a bump in blood pressure or blood sugar, mood changes, or trouble sleeping. The evidence here is fairly limited and not certain to play out the same way with hydrocortisone. Danshen can also affect blood clotting, which is its own concern. Please run this combination by your pharmacist or doctor before mixing them.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Hydrocortisone also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Your body uses a liver and gut enzyme called CYP3A4 to help break down hydrocortisone. Danshen can nudge that enzyme in two directions. Taken steadily for a week or two, it appears to speed CYP3A4 up, which could clear hydrocortisone a bit faster and make it work less well. That matters most if you take oral hydrocortisone (like Cortef) to replace low natural steroid levels, where weaker effect could leave you feeling more tired, weak, or generally unwell. A single dose of Danshen seems to do the opposite, briefly raising drug levels.
Honestly, this evidence is limited and based on a different drug (midazolam), so the real effect on hydrocortisone is uncertain. Skin creams are unlikely to be affected. Please check with your pharmacist or doctor before combining them.

Hydrocortisone comes in many products, and each one is approved for different uses. Which one you get depends on the condition being treated.
Cortef tablets are used for adrenal insufficiency, congenital adrenal hyperplasia, rheumatic disorders (such as rheumatoid arthritis and gouty arthritis) and collagen diseases such as lupus. Hydrocortisone sodium succinate and Solu-Cortef injections are used when oral therapy is not feasible, for severe allergic states, skin diseases, and adrenal insufficiency. Alkindi (oral granules) is used as replacement therapy in children with adrenocortical insufficiency. Cortenema, Hydrocortisone Rectal Suspension and Cortifoam are used for ulcerative colitis or ulcerative proctitis. Topical hydrocortisone butyrate and valerate products, including Evesse lotion, are used for itchy, inflamed skin conditions and atopic dermatitis.
