Diclofenac Sodium falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Danshen interacts with.

Theoretically, danshen may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
Danshen has been reported to have antithrombotic effects. Animal research also suggests that taking a danshen combination formula with clopidogrel exhibits a synergistic increase in antiplatelet aggregation and prolongation of coagulation time when compared with either taken alone.

Here's the thing to keep in mind with diclofenac and danshen. Diclofenac is an NSAID, and like other anti-inflammatory pain relievers it can already irritate the stomach lining and make it a little harder for your blood to clot. Danshen, on its own, seems to thin the blood and slow clotting too. Put the two together and those effects can stack up, which may raise your chances of bruising, nosebleeds, or stomach bleeding (think dark or bloody stools, or stomach pain).

To be fair, this concern is based mostly on animal studies and scattered reports, not strong human trials, so it's more of a caution than a sure thing. Still, since bleeding is serious, please check with your pharmacist or doctor before pairing them, and don't change your medicine on your own.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP2C9.
In vitro research shows that various constituents of danshen inhibit the activity of CYP2C9. So far, this interaction has not been reported in humans.

Here's the simple version: your body relies on a liver enzyme called CYP2C9 to break down and clear diclofenac. Danshen, in lab and test-tube studies, appears to slow that same enzyme down. If that happened in people, diclofenac could hang around longer and build up, which may mean stronger drug effects and a higher chance of typical NSAID side effects like stomach upset, heartburn, ulcers or bleeding, fluid retention, and added strain on the kidneys or blood pressure.

I want to keep this in perspective, though. So far this is only seen in lab studies, not in actual patients, so it's mostly theoretical. One real concern is that both danshen and diclofenac can affect bleeding, so combining them isn't something I'd do without checking first. Please loop in your pharmacist or doctor before pairing the two.

  • Oral tablets: osteoarthritis, rheumatoid arthritis, and (delayed-release) ankylosing spondylitis, plus period pain and mild to moderate pain for the immediate-release potassium tablets.
  • Capsules: mild to moderate acute pain in adults.
  • Oral solution (powder for solution): acute treatment of migraine attacks in adults.
  • Topical gel and solution: osteoarthritis pain, mainly of the knees (and hands for the gel).
