CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Rifampin
Rifampin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Danshen interacts with.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the wrinkle with this pairing: Rifampin is itself one of the strongest enzyme boosters out there, including for CYP3A4, the same pathway Danshen seems to nudge. So you're really stacking two things that can speed up how the body clears certain medications. The bigger concern usually isn't what Danshen does to Rifampin, it's that both of them can lower the levels of other drugs you take, like birth control, some heart medicines, or transplant medicines.
The evidence here is fairly early (small studies in healthy people, plus some mixed lab results), so I wouldn't panic. Still, because Rifampin is taken to fully clear a serious infection like TB, you don't want anything throwing off your levels. Please check with your pharmacist or doctor before adding Danshen, rather than starting or stopping anything on your own.

What Is Rifampin Used For?
Brand names: Rifadin, Rimactane, Rifadin Injection, Rofactor
Rifampin treats all forms of tuberculosis. It is always used as part of a combination of TB medicines, not on its own.
It also treats asymptomatic meningococcal carriers. These are people who carry the bacteria Neisseria meningitidis in the nose and throat without being sick. Rifampin is not used to treat an actual meningococcal infection.
Rifampin capsules are taken by mouth. Rifadin IV and Rifampin for Injection are given through a vein for the initial treatment and retreatment of tuberculosis when the medicine cannot be taken by mouth.
