Why this applies to Tacrolimus

Tacrolimus falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the key thing about tacrolimus: your body relies on an enzyme called CYP3A4 to break it down, and the level in your blood has to stay in a fairly narrow window. Too low, and your transplanted organ could be at risk of rejection. Too high, and you can run into side effects like kidney trouble, tremors, headaches, high blood pressure, or elevated blood sugar.

Danshen seems to nudge this enzyme in both directions depending on how it's taken. Used steadily for a couple of weeks, it tends to speed up clearance, which could push your tacrolimus levels down. A single dose may briefly raise them. Either way, that's an unwelcome wobble for a medicine this sensitive. The evidence here is early and modest, but please don't add Danshen without talking to your transplant team or pharmacist first.

Tacrolimus is prescribed to help prevent your body from rejecting a transplanted organ. It is used in adults who receive a kidney, liver, or heart transplant, and in children who receive a liver transplant. It is always used alongside other anti-rejection medicines.
