Ciclesonide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the wrinkle with this pair. Ciclesonide is actually a prodrug. The inhaled medicine itself is fairly inactive until enzymes in your airways turn it into its active form, and CYP3A4 in your body then helps clear it out. Danshen can nudge that same CYP3A4 enzyme, and which direction depends on timing. In one small study, taking Danshen for about 10 to 14 days actually sped up CYP3A4, while a single dose briefly slowed it.

For an inhaled steroid like ciclesonide, this is a low-key concern. Most of the drug stays in your lungs, and very little reaches your bloodstream, so changes in body-wide clearance usually won't change how well it controls your asthma. The evidence here is also limited. Still, it's worth a quick word with your pharmacist before adding Danshen.

What Is Ciclesonide Oral Inhalation Used For?

Brand names: Omnaris, Alvesco, Zetonna

Alvesco (ciclesonide) inhaler is approved for the maintenance treatment of asthma as a preventive therapy in adults and children 12 years of age and older. It is taken regularly to reduce airway inflammation and help prevent asthma symptoms — not to relieve a sudden attack.

Alvesco is not a rescue inhaler and should not be used to treat sudden wheezing or acute asthma episodes. It is also not approved for children under 12.
