Danshen and Diphenhydramine Hydrochloride, Naproxen Sodium

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Danshen interacts with whole categories of medication. Diphenhydramine Hydrochloride, Naproxen Sodium falls into 3 of those categories, so it can interact with Danshen in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Major).

Major

ANTICOAGULANT/ANTIPLATELET DRUGS

1 of 3

Why this applies to Diphenhydramine Hydrochloride, Naproxen Sodium

Diphenhydramine Hydrochloride, Naproxen Sodium falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Danshen interacts with.

Theoretically, danshen may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
Danshen has been reported to have antithrombotic effects. Animal research also suggests that taking a danshen combination formula with clopidogrel exhibits a synergistic increase in antiplatelet aggregation and prolongation of coagulation time when compared with either taken alone.
Severity Major Likelihood Probable Evidence D

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Aleve PM combines naproxen, an anti-inflammatory pain reliever, with diphenhydramine, the antihistamine that makes it good for sleep. The piece that matters here is the naproxen. Like other NSAIDs, naproxen can irritate the stomach lining and make your platelets a little less sticky, so it already nudges your bleeding risk up a bit on its own.

Danshen has been reported to thin the blood too. Put the two together and the concern is added bleeding risk, things like stomach upset that turns into an ulcer, easy bruising, nosebleeds, or dark stools. Honestly, this is mostly theoretical and based on weak, mostly animal evidence, so don't panic. Still, since both can affect clotting, it's worth a quick chat with your pharmacist or doctor before combining them, especially if you take them regularly.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

2 of 3

Diphenhydramine Hydrochloride, Naproxen Sodium also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research shows that danshen tincture and various constituents of danshen inhibit the activity of CYP1A2. However, this activity has not been shown in humans when theophylline, a CYP1A2 substrate, was used as a target drug.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Aleve PM combines naproxen (a pain reliever) with diphenhydramine (the sleep aid in Benadryl). The concern here is mostly theoretical. In lab studies, Danshen can slow down a liver enzyme called CYP1A2, which helps break down certain medicines. If that enzyme is blocked, those drugs could build up a bit and their effects might feel stronger. With this combo, that could mean extra grogginess, dry mouth, or dizziness from the diphenhydramine side.

Here's the reassuring part: when researchers actually tested this in people, Danshen did not meaningfully change a CYP1A2 drug. So real-world risk looks low. There's also a separate point worth mentioning: Danshen may thin the blood, and naproxen can irritate the stomach, so together they could raise bleeding risk. Please check with your pharmacist or doctor before combining them.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

3 of 3

Diphenhydramine Hydrochloride, Naproxen Sodium also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP2C9.
In vitro research shows that various constituents of danshen inhibit the activity of CYP2C9. So far, this interaction has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Aleve PM combines naproxen (an anti-inflammatory pain reliever) with diphenhydramine (a sedating antihistamine that helps you sleep). The naproxen part is cleared by a liver enzyme called CYP2C9. In lab tests, certain compounds in Danshen appear to slow that enzyme down. If that happened in real life, naproxen could build up a bit and lead to stronger effects like stomach upset, heartburn, or in some cases stomach irritation or bleeding.

Here's the honest part: this is only seen in test tube and animal studies, never confirmed in people, so I wouldn't lose sleep over it. There's also a separate concern worth mentioning, since Danshen may thin the blood and naproxen can irritate the stomach lining, the two together could nudge bleeding risk up. Please run this combo by your pharmacist or doctor before mixing them.

Danshen is a root used in traditional Chinese medicine, mainly for heart and circulation problems.
