Nintedanib Esylate falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Danshen interacts with.

Theoretically, danshen may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
Danshen has been reported to have antithrombotic effects. Animal research also suggests that taking a danshen combination formula with clopidogrel exhibits a synergistic increase in antiplatelet aggregation and prolongation of coagulation time when compared with either taken alone.

Here's the thing to keep in mind with Ofev (nintedanib): bleeding is already a known side effect of this medicine, even on its own. Danshen has been reported to slow blood clotting and act a bit like a blood thinner. So putting the two together could stack their effects and raise your risk of bruising, nosebleeds, bleeding gums, or harder-to-stop bleeding from a cut.

I want to be straight with you: this concern is mostly theoretical and based on weak, anecdotal and animal evidence, not solid human studies. Even so, because nintedanib carries its own bleeding risk, I'd play it safe. Please check with your pharmacist or doctor before combining them, and let them know right away if you notice unusual bleeding or bruising.

Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the gist for Ofev (nintedanib). Your body relies on a kind of cellular pump called P-glycoprotein to move nintedanib along and help clear it. Danshen seems to slow that pump down a bit. In one human study, danshen raised blood levels of another P-gp drug by roughly a third, so the same thing could happen here, meaning more nintedanib hanging around than expected.

For nintedanib, higher levels usually mean more of its typical side effects, especially diarrhea, nausea, vomiting, loss of appetite, and possibly liver irritation. The evidence is moderate, not ironclad, so don't panic. Just please loop in your pharmacist or doctor before combining the two, and don't change your Ofev dose on your own.

Nintedanib Esylate also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the gist: Ofev (nintedanib) is partly handled by a gut enzyme called CYP3A4. Danshen can affect that same enzyme. When taken regularly for a week or two, danshen seems to speed up CYP3A4, which could lower how much nintedanib stays in your body. That might make it work less well at slowing your lung scarring. Oddly, a single dose of danshen seems to do the opposite and push levels up, which could mean more diarrhea, nausea, or liver irritation.

This is based on early research using a different drug, so the real effect on Ofev isn't proven. Still, since Ofev is an important and sometimes tricky medicine, please check with your pharmacist or doctor before adding danshen rather than starting or stopping anything on your own.

What Is Nintedanib Used For?

Brand name: Ofev

Nintedanib capsules are approved for adults with idiopathic pulmonary fibrosis (IPF) — a chronic, progressive scarring of the lungs — and for adults with chronic fibrosing interstitial lung diseases (ILDs) with a progressive phenotype, meaning fibrosing lung diseases that continue to worsen over time despite treatment.

Nintedanib capsules (including Ofev) are also indicated to slow the rate of decline in lung function in adults with systemic sclerosis-associated interstitial lung disease (SSc-ILD), a lung complication of the autoimmune condition systemic sclerosis (scleroderma). Note that the Nintedanib esylate capsule product is approved for IPF and chronic fibrosing ILDs with a progressive phenotype, but SSc-ILD approval is carried by Ofev and other nintedanib capsule products — always confirm with your prescriber which product you have been prescribed.
