Erythromycin falls under P-GLYCOPROTEIN SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels of drugs cleared by p-glycoprotein.
Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the gist for erythromycin. Your body uses a kind of cellular pump called P-glycoprotein to help move this antibiotic in and out of cells. Danshen seems to slow that pump down. In one small study it raised levels of a similar pumped drug by about a third. So with erythromycin, the worry is that more of it could hang around in your system than expected.

Erythromycin is not a prodrug, so higher levels would mean stronger effects, including its usual side effects: stomach cramps, nausea, or diarrhea. Rarely, high levels can affect heart rhythm. The evidence here is limited and only suggests this is possible, not certain. Please run it by your pharmacist or doctor before combining them.

Erythromycin also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the gist for erythromycin: your body partly clears it using a liver and gut enzyme called CYP3A4. Danshen seems to nudge that enzyme in different directions depending on how you take it. Taken regularly for a week or two, danshen appears to speed up CYP3A4, which could lower erythromycin levels and make it less effective at knocking out your infection. Interestingly, a single dose of danshen did the opposite in studies, briefly raising drug levels.

Erythromycin is not a prodrug, so it works as-is, and weaker blood levels can mean a partly treated infection. The evidence here is early and modest, but when you're on an antibiotic, you really want it working at full strength. Please run this combo past your pharmacist or doctor before mixing them.

About Erythromycin

What Is Erythromycin Used For?

Brand names: Erythro S, PCE, E-Mycin, Erythro-ES, EryPed Chewable, Eryc, Ery-Tab, Rythro Enca, Erythro E-C, Erythro Base, Ilosone, Ilotycin

Erythromycin treats bacterial infections caused by susceptible organisms. Oral and intravenous forms are used for upper and lower respiratory tract infections, skin and soft-tissue infections, pertussis (whooping cough), Legionnaires' disease, sexually transmitted infections (gonorrhea, syphilis, chlamydia), listeriosis, and other conditions. Erythrocin Lactobionate IV and erythromycin lactobionate injection are specifically for patients who cannot take oral medication or who need rapid, high blood levels due to severe infection — IV therapy should be switched to oral as soon as appropriate.

Erythromycin topical solution 2% and Ery 2% Pads (swabs) are indicated only for the topical treatment of acne vulgaris (acne) and are applied directly to the skin — they are not used for internal infections.
