Erythromycin falls under CYTOCHROME P450 1A2 (CYP1A2) INHIBITORS — the group Green Tea interacts with.

Theoretically, concomitant use might increase the levels and adverse effects of caffeine.
Green tea contains caffeine. Caffeine is metabolized by cytochrome P450 1A2 (CYP1A2),. Theoretically, drugs that inhibit CYP1A2 may decrease the clearance rate of caffeine from green tea and increase caffeine levels.

Here's a flip on what you might expect. With this combination, it's actually the caffeine in your green tea that's the focus, not the erythromycin itself. Erythromycin can slow down a liver enzyme (CYP1A2) that your body uses to clear caffeine. So if you're drinking green tea or taking green tea extract while on this antibiotic, the caffeine may hang around longer and build up a bit.

In real life, that could mean feeling more jittery, a racing or pounding heart, trouble sleeping, or an upset stomach. The good news is this is a theoretical interaction based on how the drug works, not strong proof, and a normal cup or two of green tea is unlikely to cause trouble. If you notice these effects, just ease up on the caffeine and check with your pharmacist.

Green tea might increase the levels and adverse effects of P-glycoprotein (P-gp) substrates.
In vitro research and case reports suggest that green tea inhibits drug efflux by P-gp, potentially increasing serum levels of P-gp substrates. Case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking green tea and certain P-gp substrates.

Here's the gist: your body uses a kind of cellular bouncer called P-glycoprotein (P-gp) to push certain drugs back out and keep their levels in check. Erythromycin is one of those drugs that P-gp helps move along. Green tea may slow that bouncer down a little, which in theory could let a bit more erythromycin linger in your system.

If levels nudged up, you might notice more of erythromycin's usual side effects, like stomach upset, nausea, cramping, or diarrhea. That said, this concern rests on lab studies and a few scattered case reports, so it's far from certain. A cup or two of brewed green tea is unlikely to cause trouble; concentrated extract pills are the bigger question. If you take those, just give your pharmacist or doctor a heads-up.

Erythromycin also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Green tea is unlikely to produce clinically significant changes in the levels and clinical effects of CYP3A4 substrates.
In vitro and in vivo research suggests that green tea can inhibit intestinal CYP3A and induce hepatic CYP3A4 enzymes. However, this effect is unlikely to be clinically significant, as green tea does not appear to affect CYP3A4 activity in humans.

Here's the short version: if you're sipping green tea while taking erythromycin, you really don't need to worry. Erythromycin is handled by an enzyme called CYP3A4, and lab and animal studies hint that green tea might nudge that enzyme a little, either slowing it in the gut or speeding it up in the liver. But in actual people, green tea doesn't seem to change CYP3A4 activity in any meaningful way.

So in plain terms, your normal cup or two of tea is very unlikely to make erythromycin stronger (more stomach upset, nausea, or diarrhea) or weaker. This is theoretical, low-quality evidence. If you're using concentrated green tea extract pills, it's still fair to mention them to your pharmacist, just to be safe.

About Erythromycin

What Is Erythromycin Used For?

Brand names: Erythro S, PCE, E-Mycin, Erythro-ES, EryPed Chewable, Eryc, Ery-Tab, Rythro Enca, Erythro E-C, Erythro Base, Ilosone, Ilotycin

Erythromycin treats bacterial infections caused by susceptible organisms. Oral and intravenous forms are used for upper and lower respiratory tract infections, skin and soft-tissue infections, pertussis (whooping cough), Legionnaires' disease, sexually transmitted infections (gonorrhea, syphilis, chlamydia), listeriosis, and other conditions. Erythrocin Lactobionate IV and erythromycin lactobionate injection are specifically for patients who cannot take oral medication or who need rapid, high blood levels due to severe infection — IV therapy should be switched to oral as soon as appropriate.

Erythromycin topical solution 2% and Ery 2% Pads (swabs) are indicated only for the topical treatment of acne vulgaris (acne) and are applied directly to the skin — they are not used for internal infections.
