Green Tea and Levonorgestrel

These two interact in 4 distinct ways. Each one is explained separately below.

Why Are There 4 Interactions?

Green Tea interacts with whole categories of medication. Levonorgestrel falls into 4 of those categories, so it can interact with Green Tea in 4 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

HEPATOTOXIC DRUGS

1 of 4

Why this applies to Levonorgestrel

Levonorgestrel falls under HEPATOTOXIC DRUGS — the group Green Tea interacts with.

Theoretically, concomitant use might have additive adverse hepatotoxic effects.
Green tea extract supplements have been linked to several cases of hepatotoxicity and might have additive hepatotoxic effects with other drugs..
Severity Moderate Likelihood Unlikely Evidence D

Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for levonorgestrel (like Plan B or your Mirena IUD): the concern listed isn't about green tea changing how well your birth control works. Instead, it's about a possible added strain on the liver. Both the medication and concentrated green tea extracts can, in rare cases, stress liver cells, so taking them together might theoretically add up.

I want to be honest about how shaky this one is. It's based on scattered anecdotes, not solid studies, and it's rated unlikely to actually happen. A regular cup or two of brewed green tea is very unlikely to matter at all. The bigger question marks come with high-dose green tea extract pills. If you ever notice yellowing skin, dark urine, or belly pain, check in with your pharmacist or doctor.

CYTOCHROME P450 1A2 (CYP1A2) INHIBITORS

2 of 4

Levonorgestrel also falls under CYTOCHROME P450 1A2 (CYP1A2) INHIBITORS.

Theoretically, concomitant use might increase the levels and adverse effects of caffeine.
Green tea contains caffeine. Caffeine is metabolized by cytochrome P450 1A2 (CYP1A2),. Theoretically, drugs that inhibit CYP1A2 may decrease the clearance rate of caffeine from green tea and increase caffeine levels.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to understand about this pairing: it's really about the caffeine in green tea, not so much the levonorgestrel itself. Levonorgestrel (think Plan B or Mirena) can mildly block an enzyme called CYP1A2, which is the same enzyme your body uses to clear caffeine. So in theory, sipping caffeinated green tea while on this hormone could let caffeine linger a little longer, leaving you feeling more jittery, with a racing heart, trouble sleeping, or an upset stomach.

I'll be honest with you: this is a theoretical concern based on how these things work on paper, not from strong real-world studies. For most people it's nothing to lose sleep over. If you notice extra caffeine jitters, just cut back on the tea, and check with your pharmacist if you have questions.

CONTRACEPTIVE DRUGS

3 of 4

Levonorgestrel also falls under CONTRACEPTIVE DRUGS.

Theoretically, concomitant use might increase the effects and adverse effects of caffeine found in green tea.
Green tea contains caffeine. Oral contraceptives can decrease caffeine clearance by 40% to 65%.
Severity Moderate Likelihood Probable Evidence B

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing about this pairing: it's really about the caffeine in green tea, not your levonorgestrel itself. The interaction runs in one direction. Hormonal contraceptives can slow down how fast your body clears caffeine, sometimes by 40% to 65%. So the green tea (or coffee, or soda) you drink may hang around longer and hit a little harder than usual.

In practice, that might mean feeling more jittery, a faster heartbeat, trouble sleeping, or feeling on edge. Keep in mind a single Plan B dose is quick and short-lived, so this matters more with ongoing options like Mirena or the daily progestin products. Note this is largely theoretical, and a cup or two of green tea is usually fine. If you're sensitive to caffeine or notice these symptoms, chat with your pharmacist.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

4 of 4

Levonorgestrel also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Green tea is unlikely to produce clinically significant changes in the levels and clinical effects of CYP3A4 substrates.
In vitro and in vivo research suggests that green tea can inhibit intestinal CYP3A and induce hepatic CYP3A4 enzymes. However, this effect is unlikely to be clinically significant, as green tea does not appear to affect CYP3A4 activity in humans.
Severity Minor Likelihood Unlikely Evidence D

Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the short version: if you're taking levonorgestrel (like Plan B for emergency contraception, or a Mirena/Skyla IUD), drinking green tea is very unlikely to cause any real problem. Levonorgestrel is partly processed by a liver enzyme called CYP3A4, and green tea can tug on that enzyme a little in lab tests, both speeding it up and slowing it down. But in actual people, green tea doesn't seem to meaningfully change CYP3A4 activity.

This concern comes from test-tube and animal studies, not from real-world cases, so I wouldn't lose sleep over your morning cup. There's no reliable evidence that green tea would weaken Plan B's ability to prevent pregnancy. If you're using concentrated green tea extract pills, it's still smart to mention it to your pharmacist or doctor, just to be safe.

About Green Tea

What is Green Tea?

Green Tea
  • Antioxidant support
  • Weight loss
  • Heart health
  • Mental alertness
  • Cancer risk reduction

Green tea is a popular beverage rich in antioxidants called catechins, and drinking it in normal amounts is considered safe for most people. Concentrated green tea extracts are a different story—they may help modestly with weight or heart health for some, but high doses have been linked to rare liver problems. Talk to your pharmacist or doctor before taking green tea supplements, especially if you have liver concerns or are pregnant.

About Levonorgestrel

What Is Levonorgestrel Used For?

Brand names: Plan B, Mirena, Levonelle One Step, Levonelle 1500, Norgeston, Skyla, Liletta, Kyleena

Levonorgestrel is used in two very different ways depending on the product. As an intrauterine system (IUS), Mirena and Liletta prevent pregnancy for up to 8 years and also treat heavy menstrual bleeding for up to 5 years in people who choose intrauterine contraception. Kyleena prevents pregnancy for up to 5 years, and Skyla prevents pregnancy for up to 3 years — neither Kyleena nor Skyla is indicated for heavy menstrual bleeding.

As an oral tablet, products such as AfterPill, AfterPlan, Amazon Basic Care Levonorgestrel, Econ Morning After, Emergency Contraceptive, and Empowered Choice are used as emergency (backup) contraception to reduce the chance of pregnancy after unprotected sex or contraceptive failure. The oral tablet is not intended for routine birth control.
