Here's the gist: this is a liver concern, not a worry about Sweet Annie making your levonorgestrel work better or worse. Levonorgestrel (like Plan B or your Mirena/Skyla device) is processed through the liver, and Sweet Annie has been linked to occasional liver irritation. The thought is that, in theory, taking both could be a little harder on your liver than either one alone.

I want to be honest about how thin the evidence is. This is based on scattered, anecdotal reports, not solid studies, so for most people it's a small, mostly theoretical concern. If you notice yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, check in with your pharmacist or doctor. Don't change anything on your own.

Levonorgestrel also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Sweet Annie may alter plasma levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research shows that the Sweet Annie constituent artemisinin induces CYP3A4, possibly increasing CYP3A4 activity by 1.9-fold. However, Sweet Annie extract seems to inhibit the activity of CYP3A4 in vitro, suggesting that other constituents of Sweet Annie play a role in its effects on the overall activity of this enzyme. More information is needed to determine whether taking Sweet Annie extract affects the metabolism of CYP3A4 substrates.

Here's the plain version: levonorgestrel (like in Plan B) gets broken down in your body partly by a liver enzyme called CYP3A4. Sweet Annie may nudge that enzyme, and that's where the worry comes in. The tricky part is the direction isn't clear. Lab studies suggest one of its compounds (artemisinin) might speed up CYP3A4, which could clear the hormone faster and make your emergency contraceptive less reliable. But the whole-plant extract seemed to do the opposite in those same lab tests.

Because this is only test-tube data, we honestly don't know how it plays out in real people. Since a failed dose here could mean an unplanned pregnancy, I'd play it safe and ask your pharmacist or doctor before combining them.

About Levonorgestrel

What Is Levonorgestrel Used For?

Brand names: Plan B, Mirena, Levonelle One Step, Levonelle 1500, Norgeston, Skyla, Liletta, Kyleena

Levonorgestrel is used in two very different ways depending on the product. As an intrauterine system (IUS), Mirena and Liletta prevent pregnancy for up to 8 years and also treat heavy menstrual bleeding for up to 5 years in people who choose intrauterine contraception. Kyleena prevents pregnancy for up to 5 years, and Skyla prevents pregnancy for up to 3 years — neither Kyleena nor Skyla is indicated for heavy menstrual bleeding.

As an oral tablet, products such as AfterPill, AfterPlan, Amazon Basic Care Levonorgestrel, Econ Morning After, Emergency Contraceptive, and Empowered Choice are used as emergency (backup) contraception to reduce the chance of pregnancy after unprotected sex or contraceptive failure. The oral tablet is not intended for routine birth control.
