Why this applies to Tacrolimus

Tacrolimus falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Sweet Annie interacts with.

Sweet Annie may alter plasma levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research shows that the Sweet Annie constituent artemisinin induces CYP3A4, possibly increasing CYP3A4 activity by 1.9-fold. However, Sweet Annie extract seems to inhibit the activity of CYP3A4 in vitro, suggesting that other constituents of Sweet Annie play a role in its effects on the overall activity of this enzyme. More information is needed to determine whether taking Sweet Annie extract affects the metabolism of CYP3A4 substrates.

Here's the situation with tacrolimus (Prograf) and Sweet Annie. Your body clears tacrolimus using a liver enzyme called CYP3A4, and Sweet Annie may bump that enzyme either way. In lab dishes, its main compound artemisinin sped the enzyme up (which could lower your tacrolimus levels and risk organ rejection), while the whole-plant extract slowed it down (which could raise levels and bring on side effects like kidney trouble, tremor, headache, high blood pressure, or high blood sugar).

Tacrolimus is not a prodrug, and it has a narrow safety window, so even small shifts matter. The good news: this is only test-tube data, so we honestly don't know how it plays out in people. Please loop in your transplant team before adding it.

Tacrolimus is prescribed to help prevent your body from rejecting a transplanted organ. It is used in adults who receive a kidney, liver, or heart transplant, and in children who receive a liver transplant. It is always used alongside other anti-rejection medicines.

Because tacrolimus works by dampening your immune system, it lowers the risk of your body attacking the new organ — but it also means you need to be watched closely for infections and other side effects throughout your treatment.
