HEPATOTOXIC DRUGS
Why this applies to Methotrexate
Methotrexate falls under HEPATOTOXIC DRUGS — the group Sweet Annie interacts with.
There is some concern that Sweet Annie can adversely affect the liver.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the concern with pairing Sweet Annie and methotrexate. This isn't about one changing the other's blood levels. Instead, both can be tough on your liver, and the worry is that the strain could add up. Methotrexate already carries a known risk of liver irritation, which is why your doctor checks your liver enzymes with regular blood tests.
I want to be honest about the strength of this: it's mostly theoretical and based on scattered reports, not solid studies, so don't panic if you've taken both. Still, since methotrexate's liver effects are well documented, I'd play it safe. Watch for things like unusual fatigue, belly pain, dark urine, or yellowing of the skin or eyes, and run any Sweet Annie use past your pharmacist or doctor first.

From our pharmacist’s overview on the Methotrexate page
“The liver is where methotrexate and supplements clash most. Methotrexate (Rheumatrex, Otrexup, Rasuvo), used for rheumatoid arthritis, psoriasis, and certain cancers, can strain the liver on its own, and 38 of the 82 interacting ingredients on this list add liver risk of their own, alcohol included, which makes this a list to approach…”

About Sweet Annie
What is Sweet Annie?
- Malaria (traditional)
- Fever
- Inflammation
- Digestive complaints
- Antimicrobial support
- Insect repellent
Sweet Annie (Artemisia annua) is the source of artemisinin, a compound used in prescription antimalarial drugs. While the purified drug is well studied for malaria, the herb itself as a supplement is not proven for the wide range of conditions people use it for, and self-treating malaria with the raw herb is dangerous. Talk to a pharmacist or doctor before using it.
About Methotrexate
What Is Methotrexate Used For?
Brand names: Rheumatrex, Otrexup, Rasuvo, Reditrex
Methotrexate treats a wide range of serious conditions. In cancer care, it is used for acute lymphoblastic leukemia, non-Hodgkin lymphoma, osteosarcoma, breast cancer, head and neck cancer, and gestational trophoblastic neoplasia (a rare tumor related to pregnancy). It is also used to prevent and treat meningeal leukemia (cancer affecting the lining of the brain and spinal cord).
Outside of cancer, methotrexate is prescribed for adults with rheumatoid arthritis (RA), children with polyarticular juvenile idiopathic arthritis (a form of childhood arthritis), and adults with severe psoriasis that hasn't responded well to other treatments.

Methotrexate Is Linked to Lower Levels of One Nutrient
Natural Medicines records Methotrexate as a medicine that can lower these. This is not an interaction with Sweet Annie — it is a separate effect of the medicine, and it is a reason some people are advised to supplement rather than to avoid one:
