Dextromethorphan falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Sweet Annie interacts with.

Sweet Annie may alter plasma levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research shows that the Sweet Annie constituent artemisinin induces CYP3A4, possibly increasing CYP3A4 activity by 1.9-fold. However, Sweet Annie extract seems to inhibit the activity of CYP3A4 in vitro, suggesting that other constituents of Sweet Annie play a role in its effects on the overall activity of this enzyme. More information is needed to determine whether taking Sweet Annie extract affects the metabolism of CYP3A4 substrates.

Here's the honest picture with Sweet Annie and your cough medicine. Dextromethorphan is partly handled by a liver enzyme called CYP3A4, and Sweet Annie may tug on that enzyme. The catch is the lab data points in two directions at once: one piece suggests it speeds the enzyme up (which could weaken your cough relief a bit), while the whole-plant extract seemed to slow it down (which could let more dextromethorphan linger, nudging up drowsiness or dizziness).

So we honestly can't say which way it lands in a real person. This is all test-tube research, not human studies, so I wouldn't lose sleep over it. Dextromethorphan is also a short-term cough remedy, not a daily must-have. Still, if you take Sweet Annie regularly, give your pharmacist a quick heads-up before combining them.

What Is Dextromethorphan Used For?

Brand names: Hold, Benylin Adult, Benylin Pediatric, Theraflu Thin Strips Long Acting Cough, Neo Citran Thin Strips Cough

Dextromethorphan temporarily relieves cough due to minor throat and bronchial irritation, such as the kind that comes with the common cold or breathing in irritants. It also helps reduce the urge to cough so you — or your child — can get to sleep.

Some products, including CVS Long Acting Cough and Premier Value Tussin Cough oral liquids, are also indicated to help loosen phlegm (mucus) and thin bronchial secretions to help drain the airways.
