Here's the basic concern with pairing Sweet Annie and nitrofurantoin: both can be tough on your liver. Nitrofurantoin, on rare occasions, can irritate the liver on its own, and there's some worry that Sweet Annie (the herb form) may do the same. Put them together and the thought is the liver stress could add up.

I want to be honest about how solid this is though. This warning is mostly theoretical and rests on anecdotal reports, not strong studies. So it's a reasonable caution, not a red alert. Still, if you ever notice yellowing of your skin or eyes, dark urine, belly pain, or unusual tiredness, stop and call your doctor. Before combining these, it's worth a quick chat with your pharmacist or physician.

Sweet Annie (Artemisia annua) is the source of artemisinin, a compound used in prescription antimalarial drugs. While the purified drug is well studied for malaria, the herb itself as a supplement is not proven for the wide range of conditions people use it for, and self-treating malaria with the raw herb is dangerous. About Nitrofurantoin

What Is Nitrofurantoin Used For?

Brand names: Macrodantin, Furadantin, Furatoin, Macrobid

Nitrofurantoin is used to treat urinary tract infections (UTIs) caused by certain susceptible bacteria. The capsule forms (including Macrodantin) are indicated for UTIs due to susceptible strains of Escherichia coli, enterococci, Staphylococcus aureus, and certain strains of Klebsiella and Enterobacter species. Nitrofurantoin oral suspension (Furadantin) is indicated for adults and children 1 month of age and older for the same types of UTIs.

Nitrofurantoin is not used for kidney infections (pyelonephritis) or infections of the tissue around the kidney (perinephric abscesses). Because it works primarily in the urine and does not spread widely through body tissues, it may not fully clear the infection in some patients, and the UTI may return after treatment.
