Why this applies to Hydrocortisone

Hydrocortisone falls under P-GLYCOPROTEIN SUBSTRATES — the group Green Tea interacts with.

Green tea might increase the levels and adverse effects of P-glycoprotein (P-gp) substrates.
In vitro research and case reports suggest that green tea inhibits drug efflux by P-gp, potentially increasing serum levels of P-gp substrates. Case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking green tea and certain P-gp substrates.

Here's the idea behind this one. Your body uses a little cellular pump called P-glycoprotein (P-gp) to move certain drugs out of cells and help clear them. Green tea may gently block that pump, and since hydrocortisone is moved around by P-gp, the theory is that blocking it could let a bit more hydrocortisone build up in your system.

If that happened, you might notice more of this steroid's usual effects, such as fluid retention, a higher blood sugar, mood or sleep changes, or stomach upset. That said, the evidence here is weak and mostly anecdotal, and a few cups of green tea are far less concentrated than supplement extracts. I wouldn't lose sleep over an occasional cup, but check with your pharmacist or doctor before using green tea pills.

Hydrocortisone also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Green tea is unlikely to produce clinically significant changes in the levels and clinical effects of CYP3A4 substrates.
In vitro and in vivo research suggests that green tea can inhibit intestinal CYP3A and induce hepatic CYP3A4 enzymes. However, this effect is unlikely to be clinically significant, as green tea does not appear to affect CYP3A4 activity in humans.

Here's the short version: drinking green tea alongside your hydrocortisone is very unlikely to cause any real problem. Your liver uses an enzyme called CYP3A4 to help break hydrocortisone down, and lab studies suggest green tea might nudge that enzyme one way or the other. But in actual people, green tea doesn't seem to meaningfully change how this enzyme works.

So you don't need to worry about your normal cup of tea making hydrocortisone too strong (more puffiness, mood changes, raised blood sugar) or too weak. This is based only on test-tube and animal data, so the evidence is weak. If you're taking concentrated green tea extract pills, just mention it to your pharmacist to be safe.

What Is Hydrocortisone Used For?

Brand names: Cortef, Hydrocortone, Carmol HC cream, Hydrocortisone Iodoquinol Cream, Vytone Cream, Massengil Medicated Towelettes, Dioderm, Efcortelan Cream, Mildison, Alphaderm, Calmurid HC, Efcortelan Ointment, Texacort, Alkindi Sprinkle

Hydrocortisone is used to calm skin inflammation and relieve itching caused by conditions that respond to corticosteroid treatment. In adults, it treats a broad range of inflammatory skin conditions. In children as young as 3 months old, it is specifically approved for mild to moderate atopic dermatitis (eczema).

It is also used in rectal foam form to treat inflammation of the lower bowel, such as ulcerative proctitis.
