Here's the concern in plain terms: methotrexate can be hard on the liver, especially when you take it week after week for things like rheumatoid arthritis or psoriasis. That's why your doctor checks your liver enzymes with blood tests. Concentrated green tea extract supplements have, in rare cases, also been tied to liver injury. Put the two together and the worry is that the strain on your liver could add up.

I want to be honest about how strong this evidence is: it's theoretical and based on scattered case reports, so it's unlikely for most people. Drinking a normal cup or two of green tea is very different from swallowing high-dose extract capsules. Before starting any green tea supplement, run it by your pharmacist or doctor so they can keep an eye on your liver numbers.

Methotrexate also falls under ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP).

Theoretically, green tea might reduce the absorption of organic anion-transporting polypeptide (OATP) substrates.
OATPs are expressed in the small intestine and liver and are responsible for the uptake of drugs and other compounds. Research shows that two of the major catechins found in green tea, epicatechin gallate (ECG) and epigallocatechin gallate (EGCG), inhibit OATPs, specifically OATP1A2, OATP1B1, and OATP2B1. In addition, green tea has been shown to reduce the absorption of some drugs that are OATP substrates, including lisinopril, and celiprolol.

Here's the idea behind this one. The catechins in green tea (especially EGCG) can block transporters called OATPs that help shuttle certain drugs into your cells. Methotrexate uses transporters like these to get where it needs to go, so in theory green tea might lower how much methotrexate your body takes in, possibly making it work a little less well for your arthritis, psoriasis, or other condition.

I want to be straight with you, though: this is based only on lab and animal research, not on real patients taking methotrexate with green tea. So it's more of a 'good to know' than a red alert. Drinking a normal cup or two is unlikely to be a problem. Concentrated green tea extract pills are where I'd be more cautious. Methotrexate is a serious medicine, so check with your pharmacist or doctor before adding supplements.

What Is Methotrexate Used For?

Brand names: Rheumatrex, Otrexup, Rasuvo, Reditrex

Methotrexate treats a wide range of serious conditions. In cancer care, it is used for acute lymphoblastic leukemia, non-Hodgkin lymphoma, osteosarcoma, breast cancer, head and neck cancer, and gestational trophoblastic neoplasia (a rare tumor related to pregnancy). It is also used to prevent and treat meningeal leukemia (cancer affecting the lining of the brain and spinal cord).

Outside of cancer, methotrexate is prescribed for adults with rheumatoid arthritis (RA), children with polyarticular juvenile idiopathic arthritis (a form of childhood arthritis), and adults with severe psoriasis that hasn't responded well to other treatments.
