Here's the gist for green tea and nifedipine: the concern isn't about your blood pressure medicine working too strongly or too weakly. It's really about the liver. Both nifedipine (rarely) and concentrated green tea extracts have been tied to occasional liver irritation, so the worry is that stacking them might add a little extra strain on the liver.

I want to be honest about how shaky this is. It's theoretical, based on a handful of anecdotal reports, and considered unlikely to happen. A cup or two of brewed green tea is almost certainly a non-issue. High-dose green tea extract capsules are where caution makes sense. If you ever notice yellowing skin, dark urine, belly pain, or unusual tiredness, check in with your doctor or pharmacist.

Nifedipine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Green tea is unlikely to produce clinically significant changes in the levels and clinical effects of CYP3A4 substrates.
In vitro and in vivo research suggests that green tea can inhibit intestinal CYP3A and induce hepatic CYP3A4 enzymes. However, this effect is unlikely to be clinically significant, as green tea does not appear to affect CYP3A4 activity in humans.

Here's the good news if you enjoy your daily cup: green tea and nifedipine aren't likely to cause you trouble. Nifedipine is processed by an enzyme called CYP3A4, and green tea can tug on that enzyme a little in lab dishes. But in real people, studies haven't shown a meaningful change in how nifedipine works.

In theory, if green tea slowed nifedipine's breakdown, you might see more of its effects (think dizziness, flushing, headache, or swollen ankles). The evidence for this is weak, based on test-tube and animal data, so normal tea drinking shouldn't worry you. Concentrated green tea extract pills are a bigger unknown, so check with your pharmacist before adding those.

About Nifedipine

What Is Nifedipine Used For?

Brand names: Procardia XL, Adalat PA, Adalat, Nifedin, Nifed PA, Nifed, Procardia, Adalat XL, Adalat CC, Adalat LA, Adalat Retard, Adipine MR, Adipine XL, Cardilate MR, Coracten SR, Coracten XL, Fortipine LA 40, Hypolar Retard 20, Nifedipress MR, Nifopress Retard, Tensipine MR

Nifedipine treats high blood pressure (hypertension), chronic stable angina (chest pain brought on by physical effort), and vasospastic angina (chest pain caused by spasm of the coronary arteries, sometimes called Prinzmetal's angina). Lowering blood pressure with nifedipine helps reduce the risk of strokes and heart attacks.

The capsule form is approved for vasospastic angina and chronic stable angina only — it is not approved for high blood pressure. The extended-release tablet (including Procardia) covers all three uses: vasospastic angina, chronic stable angina, and high blood pressure.
