Here's the thing to know about bosentan: it can be tough on the liver all on its own. In fact, anyone taking it needs regular blood tests to keep an eye on liver enzymes. That's why this pairing is worth a mention. Concentrated green tea extract supplements (not your everyday cup of tea) have been tied to rare cases of liver injury too. Put two liver stressors together and, in theory, the risk could add up.

I want to be honest that this is based on weak, anecdotal evidence, so it's considered unlikely. Enjoying a normal cup or two of green tea is very different from high-dose pills. Still, watch for yellowing skin or eyes, dark urine, nausea, or unusual fatigue, and check with your pharmacist or doctor before adding any green tea supplement.

Bosentan also falls under ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP).

Theoretically, green tea might reduce the absorption of organic anion-transporting polypeptide (OATP) substrates.
OATPs are expressed in the small intestine and liver and are responsible for the uptake of drugs and other compounds. Research shows that two of the major catechins found in green tea, epicatechin gallate (ECG) and epigallocatechin gallate (EGCG), inhibit OATPs, specifically OATP1A2, OATP1B1, and OATP2B1. In addition, green tea has been shown to reduce the absorption of some drugs that are OATP substrates, including lisinopril, and celiprolol.

Here's the gist with bosentan (Tracleer) and green tea. Your body relies on tiny transporters called OATPs to help pull bosentan into the gut and liver. Lab studies suggest the catechins in green tea (especially EGCG) can block those transporters. In theory, that could mean a bit less bosentan gets absorbed, which might make it work less well for your pulmonary arterial hypertension. Since bosentan helps keep the lung blood vessels open and improves how well you can exercise, weaker absorption isn't something you want.

The important caveat: this is based on test-tube and animal research, not real patient studies, so it's more of a theoretical concern. A cup or two of brewed green tea is unlikely to be a problem. Concentrated green tea extract pills are where I'd be more cautious. Please check with your pharmacist or doctor before adding any green tea supplement.

In vitro and in vivo research suggests that green tea can inhibit intestinal CYP3A and induce hepatic CYP3A4 enzymes. However, this effect is unlikely to be clinically significant, as green tea does not appear to affect CYP3A4 activity in humans.

Here's the short version on green tea and bosentan (Tracleer): there isn't much to worry about. Bosentan is handled by a liver enzyme called CYP3A4, and lab and animal studies hint that green tea might nudge this enzyme in either direction. But in actual people, green tea doesn't seem to meaningfully change how CYP3A4 works, so the effect on bosentan is considered minor and unlikely.

In plain terms, your morning cup of green tea is very unlikely to make bosentan work less well or cause extra side effects like headache, flushing, or swelling. If you're thinking about taking a concentrated green tea extract, especially in high doses, it's worth a quick chat with your pharmacist, mainly because those extracts can rarely stress the liver, and bosentan is already watched closely for liver effects.

What Is Bosentan Used For?

Brand name: Tracleer

Bosentan is used to treat pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs. In adults, it helps improve the ability to exercise and slows how quickly the disease gets worse.

In children aged 3 years and older with idiopathic (no known cause) or congenital (present from birth) PAH, bosentan is used to lower resistance in the lung blood vessels, which is expected to improve exercise ability. Tracleer is one brand approved for both adults and pediatric patients with PAH.
