These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Green Tea interacts with whole categories of medication. Fosphenytoin Sodium falls into 2 of those categories, so it can interact with Green Tea in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
HEPATOTOXIC DRUGS
1 of 2Why this applies to Fosphenytoin Sodium
Fosphenytoin Sodium falls under HEPATOTOXIC DRUGS — the group Green Tea interacts with.
Green tea extract supplements have been linked to several cases of hepatotoxicity and might have additive hepatotoxic effects with other drugs..
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Here's the gist for fosphenytoin (Sesquient). This isn't about green tea changing how the drug works in your brain. Instead, both fosphenytoin and concentrated green tea extract can, in rare cases, be hard on the liver. The worry is that stacking two liver stressors together might add up. To be clear, this is a theoretical concern based on a few scattered reports, not strong proof, so I wouldn't lose sleep over it.
Drinking a cup or two of green tea is very unlikely to matter. The real question is high-dose green tea extract pills. If you take those, watch for yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and loop in your doctor before combining them.

PHENYTOIN (Dilantin)
2 of 2Green tea contains caffeine. Animal research suggests that caffeine can decrease the anticonvulsant activity of phenytoin. The effect does not seem to be related to the seizure threshold-lowering effects of caffeine. However, the exact mechanism of this interaction is unclear.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with Sesquient (fosphenytoin). Once it's in your body, fosphenytoin is converted into phenytoin, the active seizure-control medicine, so any concern that applies to phenytoin applies here too. The worry on file is that the caffeine in green tea might slightly blunt how well phenytoin holds your seizures in check, which could theoretically raise the chance of a breakthrough seizure.
I want to be honest about how weak this signal is. It comes from animal lab work, not human studies, and the exact reason isn't clear. Because phenytoin is a drug where small changes can matter, enjoying a normal cup or two of green tea is unlikely to cause trouble, but loading up on concentrated green tea or caffeine pills is where I'd be more cautious. Run it by your pharmacist or neurologist first.

From our pharmacist’s overview on the Fosphenytoin Sodium page
“Fosphenytoin sodium (Sesquient) converts to phenytoin in the body, and phenytoin is unforgiving about blood levels. A little too low invites seizures, a little too high brings toxicity.”

About Green Tea
What is Green Tea?
- Antioxidant support
- Weight loss
- Heart health
- Mental alertness
- Cancer risk reduction
Green tea is a popular beverage rich in antioxidants called catechins, and drinking it in normal amounts is considered safe for most people. Concentrated green tea extracts are a different story—they may help modestly with weight or heart health for some, but high doses have been linked to rare liver problems. Talk to your pharmacist or doctor before taking green tea supplements, especially if you have liver concerns or are pregnant.
