ANTIHYPERTENSIVE DRUGS
1 of 3Why this applies to Amlodipine
Amlodipine falls under ANTIHYPERTENSIVE DRUGS — the group Danshen interacts with.
Animal research suggests that danshen can produce dose-dependent hypotensive effects. Furthermore, concomitant use with captopril appears to potentiate this effect.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version: amlodipine already lowers your blood pressure by relaxing your blood vessels. Danshen, in lab and animal studies, seems to do something similar and may gently push blood pressure down too. Put them together and those effects could stack up, so in theory your blood pressure might drop more than expected.
If that happened, you could feel lightheaded, dizzy, tired, or a bit unsteady, especially when you stand up quickly. I'll be honest though, this is mostly theoretical and based on animal research, not strong human studies, so it's a possibility rather than a sure thing. Amlodipine isn't a prodrug here, so the concern is simply additive blood pressure lowering. Please check with your pharmacist or doctor before combining them.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 3Amlodipine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the gist: your body clears amlodipine using an enzyme called CYP3A4, and Danshen can nudge that enzyme in either direction. With regular use over a week or two, Danshen seems to speed CYP3A4 up, which could clear amlodipine faster and leave you with weaker blood pressure control. The flip side: a single recent dose of Danshen did the opposite in studies, raising drug levels, which could mean more dizziness, flushing, headache, or ankle swelling.
Amlodipine is the active drug already (not a prodrug), so this is really about how much stays in your system. The evidence is early and based on a different drug (midazolam), so it's not certain. Please check with your pharmacist or doctor before mixing them, and don't change your dose on your own.

3 of 3In animal research, taking danshen orally in combination with amlodipine reduced blood levels of amlodipine by about 52%. This may have been due to induction of cytochrome P450 3A4 (CYP3A4) by danshen, which has been demonstrated in vitro. So far, this interaction has not been reported in humans.

Here's the gist for you. Amlodipine relaxes your blood vessels to bring your blood pressure down, and your liver clears it using an enzyme called CYP3A4. The thinking with Danshen is that it may speed up that enzyme, so your body could break down amlodipine faster and leave less of it working. In other words, your blood pressure medicine might not pull its full weight, and you could see numbers creep up or chest pain that's less controlled.
Now, the honest part: this is based on lab dishes and animal studies (one showed about a 52% drop in amlodipine levels), and it hasn't been confirmed in people. So I wouldn't panic, but I would have a quick chat with your pharmacist or doctor before adding Danshen, and keep an eye on your blood pressure if you do.

What Is Amlodipine Used For?

Amlodipine treats high blood pressure (hypertension) in adults and children aged 6–17 years. Lowering blood pressure reduces the risk of strokes and heart attacks. Sdamlo (oral solution) is specifically indicated for hypertension in adults and pediatric patients 6 years of age and older.
Amlodipine also treats coronary artery disease in adults, including chronic stable angina (chest pain with exertion), vasospastic angina (Prinzmetal's or variant angina, caused by coronary artery spasms), and documented coronary artery disease in patients without heart failure or significantly reduced heart pumping function.
