Why this applies to Diazepam

Diazepam falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Danshen interacts with.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research shows that danshen tincture and various constituents of danshen inhibit the activity of CYP1A2. However, this activity has not been shown in humans when theophylline, a CYP1A2 substrate, was used as a target drug.

Here's the gist of what we have on file for Danshen and Valium (diazepam). In test-tube studies, Danshen seems to slow down a liver enzyme called CYP1A2, which is one of the pathways that helps clear certain medicines. If that slowdown happened in real life, diazepam could build up a bit more than usual, which might leave you feeling extra drowsy, foggy, dizzy, or unsteady on your feet.

The good news is this is mostly theoretical. When researchers tested Danshen with another CYP1A2 drug in actual people, they didn't see this effect. So I wouldn't lose sleep over it, but since Danshen can also affect blood clotting and diazepam is already sedating, it's worth a quick chat with your pharmacist or doctor before combining them.

Here's the idea with Danshen and your Valium (diazepam). Your body uses a liver enzyme called CYP2C9 to help break diazepam down. In test-tube studies, compounds in Danshen seem to slow that enzyme down. If that happened in real life, diazepam could clear out of your system a bit slower and build up, which might leave you feeling more drowsy, foggy, dizzy, or unsteady than usual.

I want to be honest about the strength of this, though. So far this is only seen in lab studies, not in actual people, so it's more of a theoretical caution than a proven problem. Diazepam isn't a prodrug, so the direction here points toward stronger effects, not weaker ones. Before adding Danshen, please run it by your pharmacist or doctor so they can look at your full medication list.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the deal with Danshen and Valium (diazepam). Your body uses a liver and gut enzyme called CYP3A4 to break diazepam down. Danshen can tug on that enzyme in two directions. In one small study, taking Danshen for about 10 to 14 days actually sped up this enzyme, which could lower your diazepam levels and make it work less well. That might mean more anxiety, less muscle relaxation, or weaker seizure control.

Interestingly, a single dose of Danshen seemed to do the opposite, raising levels briefly, which could leave you extra drowsy or unsteady. The evidence here is limited, so I wouldn't panic, but please loop in your pharmacist or doctor before combining them, especially if diazepam controls your seizures.

Theoretically, danshen might affect the levels and clinical effects of drugs requiring glucuronidation.
In vitro research shows that danshen induces the expression of glucuronosyltransferases. However, it also inhibits the activity of glucuronosyltransferases, including various members of the 1A and 2B families. The extent of inhibition of a specific glucuronosyltransferase seems to be dependent on whether or not the danshen is processed via 'sweating'. This type of processing may affect the levels of constituents in danshen that alter glucuronosyltransferase activity. So far, this interaction has not been reported in humans.

Here's the short version on combining Danshen with diazepam (Valium). Part of how your body clears diazepam involves a step called glucuronidation, where the drug gets tagged and packaged for removal. Danshen seems to tug on those same enzymes, but in two directions at once: in lab studies it sped some up and slowed others down, depending on how the Danshen was processed.

What does that mean for you? Honestly, it's hard to predict. If clearance slowed, diazepam could build up and leave you more drowsy, foggy, or unsteady on your feet. But this is theoretical, seen only in test-tube and animal work, and never confirmed in real people. I wouldn't lose sleep over it, but do mention the Danshen to your pharmacist or doctor before mixing the two.

  • Diazepam is used to relieve anxiety and to control agitation caused by alcohol withdrawal. It is also used along with other medications to control muscle spasms and spasticity caused by certain neurological disorders such as cerebral palsy (condition that causes difficulty with movement and balance), paraplegia (inability to move parts of the body), athetosis (abnormal muscle contractions), and stiff-man syndrome (a rare disorder with muscle rigidity and stiffness). Diazepam is also used along with other medications to control seizures. Diazepam is in a class of medications called benzodiazepines. It works by calming abnormal overactivity in the brain.
