Treprostinil (Oral) falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Danshen interacts with.
Danshen has been reported to have antithrombotic effects. Animal research also suggests that taking a danshen combination formula with clopidogrel exhibits a synergistic increase in antiplatelet aggregation and prolongation of coagulation time when compared with either taken alone.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the thing worth knowing if you take oral treprostinil (Remodulin) and are thinking about Danshen. Treprostinil is a prostacyclin-type medicine that, along with widening blood vessels, naturally makes your platelets less sticky. Danshen has its own blood-thinning, antiplatelet-like effects. Stack the two together and those effects can add up, which may raise your bleeding risk.
What that can look like in real life: easy bruising, nosebleeds, bleeding gums, or bleeding that's slow to stop. Heads up that the evidence here is mostly theoretical and anecdotal, not from strong human studies, so don't panic. Still, since this is a major combination on paper, please check with your pharmacist or doctor before adding Danshen rather than starting or stopping anything yourself.

Animal research suggests that danshen can produce dose-dependent hypotensive effects. Furthermore, concomitant use with captopril appears to potentiate this effect.

Oral treprostinil (Remodulin) is a medicine that relaxes and widens blood vessels to ease the strain on your heart and lungs. One of its built-in effects is lowering blood pressure. Danshen, in animal studies, also appears to drop blood pressure on its own. So the worry here is simple: putting the two together could push your blood pressure down more than expected.
If that happens, you might feel dizzy, lightheaded, flushed, or even faint, especially when standing up quickly. I want to be honest though, this concern comes mostly from lab and animal research, not solid human studies, so it's more of a theoretical flag than a proven problem. Still, please check with your pharmacist or doctor before combining them rather than stopping anything on your own.

What Is Treprostinil Used For?
Brand names: Remodulin, Orenitram
Treprostinil treats pulmonary arterial hypertension (PAH; WHO Group 1) — high blood pressure in the arteries of the lungs. Depending on the product and route, it is used to improve exercise ability, delay disease progression, or reduce symptoms tied to physical activity. Remodulin and treprostinil injection products are also indicated for patients who need to transition away from another PAH medicine called epoprostenol.
Tyvaso, Tyvaso DPI, and Yutrepia (inhaled forms) are additionally approved to treat pulmonary hypertension associated with interstitial lung disease (PH-ILD; WHO Group 3) — lung-pressure problems caused by scarring or inflammation in the lung tissue itself. Orenitram (extended-release oral tablet) is specifically indicated to delay disease progression and improve exercise capacity in PAH.
