Theoretically, taking danshen with antihypertensive drugs might increase the risk of hypotension.
Animal research suggests that danshen can produce dose-dependent hypotensive effects. Furthermore, concomitant use with captopril appears to potentiate this effect.

Here's the thinking on combining Danshen with propranolol. Propranolol already lowers your blood pressure and slows your heart rate. Danshen, on its own, may relax blood vessels and nudge blood pressure down a bit too. Stack the two together, and there's a chance your pressure could drop more than you'd want. That might show up as dizziness, lightheadedness, feeling tired, or even faint, especially when you stand up quickly.

I want to be honest about the evidence here: this concern comes mostly from animal and lab studies, not solid human trials, so it's more theoretical than proven. Still, it's worth a quick chat with your pharmacist or doctor before you start Danshen, and keep an eye on how you feel if you do.

Propranolol also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research shows that danshen tincture and various constituents of danshen inhibit the activity of CYP1A2. However, this activity has not been shown in humans when theophylline, a CYP1A2 substrate, was used as a target drug.

Here's the simple version: your liver uses an enzyme called CYP1A2 to help clear propranolol from your body. In lab dishes, Danshen seems to slow that enzyme down. If that happened in real life, propranolol could build up a bit and act stronger than expected. With this drug, that might show up as a slower heart rate, lightheadedness, unusual tiredness, or feeling faint when you stand up.

The reassuring part is that this is only seen in test-tube and lab studies, and when researchers tested it in actual people using a similar drug, they didn't see a real effect. So the worry is mostly theoretical. Still, since both Danshen and propranolol touch the heart and circulation, please run it by your pharmacist or doctor before combining them.

What Is Propranolol (Cardiovascular) Used For?

Brand names: Inderal LA, Inderal, Angilol, Syprol, Inderal Injection, Half-Inderal LA, Bedranol SR, Half Beta Prograne, Beta Prograne, Lopranol LA, Slo-Pro, InnoPran XL

This intravenous (IV) form of propranolol is used in hospital or clinical settings to treat dangerous heart rhythm problems. It is generally reserved for life-threatening arrhythmias (abnormal heart rhythms) or those that occur during anesthesia.

Specific conditions it treats include fast heart rates coming from the upper chambers of the heart (supraventricular tachycardia), certain arrhythmias caused by digitalis toxicity, dangerous ventricular rhythms that haven't responded to other treatments, and fast heart rates caused by excess adrenaline-like substances during surgery.
