Ixazomib Citrate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the situation with Danshen and your Ninlaro (ixazomib). Ixazomib is cleared from your body partly by a liver and gut enzyme called CYP3A4. Danshen isn't a simple bystander here. When taken steadily for a week or two, the research suggests it can speed up that enzyme, which could lower your ixazomib levels and make your myeloma treatment work less well. Oddly, a single dose seems to do the opposite and push levels up. Ixazomib is not a prodrug, so it relies on staying at the right level to fight the cancer.

This is based on early studies using a different drug, so it's a reasonable concern rather than a proven one. Because ixazomib is a serious cancer medicine, please don't add Danshen without first running it by your oncologist or pharmacist.

What Is Ixazomib Used For?

Brand name: Ninlaro

Ninlaro (ixazomib) is used to treat multiple myeloma — a cancer of plasma cells in the bone marrow. It is always used together with two other medicines, lenalidomide and dexamethasone, and is approved for adults who have already tried at least one other treatment.

Ninlaro is not recommended as maintenance therapy or for newly diagnosed multiple myeloma in combination with lenalidomide and dexamethasone outside of controlled clinical trials.
