Why this applies to Amitriptyline

Amitriptyline falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Danshen interacts with.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research shows that danshen tincture and various constituents of danshen inhibit the activity of CYP1A2. However, this activity has not been shown in humans when theophylline, a CYP1A2 substrate, was used as a target drug.

Your body partly relies on a liver enzyme called CYP1A2 to break down amitriptyline. Danshen, in lab studies, can slow that enzyme down. If that happened in real life, amitriptyline could build up a bit more than expected, which might mean stronger effects and more of its usual side effects. Think dry mouth, constipation, blurry vision, drowsiness, dizziness when standing up, or a faster heartbeat.

Here's the reassuring part: this concern comes only from test tube and animal research, and when scientists tested a similar enzyme in actual people, danshen didn't really change things. So the worry is mostly theoretical. Still, since amitriptyline already causes those side effects, it's worth a quick chat with your pharmacist or doctor before combining them.

In vitro research shows that various constituents of danshen inhibit the activity of CYP2C9. So far, this interaction has not been reported in humans.

Here's the basic idea. Your body uses a liver enzyme called CYP2C9 to help break down amitriptyline. Lab studies suggest some compounds in Danshen can slow that enzyme down. If that happens, amitriptyline could clear out of your system a little slower and build up, which might make its effects and side effects stronger.

For amitriptyline, that could mean more drowsiness, dry mouth, constipation, blurry vision, dizziness when standing, or a faster heartbeat. I want to be honest though: this is only seen in test-tube research, not in real people, so it's more of a theoretical caution than a proven problem. Still, before you pair these two, check with your pharmacist or doctor.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the gist for amitriptyline (Elavil): your liver and gut help clear it using an enzyme called CYP3A4, among others. Danshen can nudge that enzyme in two directions. Taken steadily for a couple of weeks, it seems to speed up CYP3A4, which could lower your amitriptyline levels and make it work less well, meaning your mood symptoms might creep back. Oddly, a single dose appears to do the opposite, temporarily raising levels and possibly bumping up side effects like drowsiness, dry mouth, constipation, or dizziness.

The human evidence here is early and was measured with a different drug, so this is more of a heads-up than a sure thing. Danshen also affects blood clotting on its own. Please check with your pharmacist or doctor before combining them.
