Theoretically, taking danshen with antihypertensive drugs might increase the risk of hypotension.
Animal research suggests that danshen can produce dose-dependent hypotensive effects. Furthermore, concomitant use with captopril appears to potentiate this effect.

Here's the gist: Lercanidipine (Zanidip) is a calcium channel blocker that lowers your blood pressure by relaxing and widening your blood vessels. Danshen, in lab and animal studies, seems to lower blood pressure too. So the worry is simple addition. If both push in the same direction, your pressure could dip more than expected.

If that happens, you might feel dizzy, lightheaded, or a bit faint, especially when you stand up quickly, and you could notice flushing or a faster heartbeat. To be fair, this is mostly theoretical and based on animal data, not solid human studies, so it may not amount to much. Still, please loop in your pharmacist or doctor before combining them, and have your blood pressure checked if you do.

Lercanidipine hydrochloride also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the situation with Danshen and your lercanidipine. Lercanidipine is a calcium channel blocker that your body clears using an enzyme called CYP3A4. Danshen can nudge that enzyme in either direction depending on how it's taken. With regular daily use for a week or two, Danshen seems to speed up CYP3A4, which could clear lercanidipine faster and leave you with weaker blood pressure control. But a single dose has the opposite effect and can raise drug levels, which could mean too much, think dizziness, flushing, headache, swollen ankles, or a racing heartbeat.

This is based on limited human research, so it's not a sure thing. Since blood pressure swings either way aren't great, please check with your pharmacist or doctor before combining them.
